Summary

The Home Health and Hospice Liaison is responsible for maintaining positive relations with hospice referral sources. The Home Health and Hospice Liaison will develop and maintain knowledge of the organization's market and will expand community awareness of home care and hospice through education and promotion. They will serve as an educational resource on home health and hospice services. In addition, the Home Health and Hospice Liaison will analyze market opportunities and establish strategies to achieve referral growth. The Home Health and Hospice Liaison will provide support to MaineHealth Care at Home and CHANS Home Health & Hospice.
Required Minimum Knowledge, Skills, and Abilities (KSAs)
- Education: High school graduate. Three to five years of sales, customer service, account management experience or other similar role in healthcare is required.
- License/Certifications: Current, valid driver's license to operate a motor vehicle, with vehicle available for work covered by liability insurance as specified by agency policy. Licensed Practical Nurse or Registered Nurse highly desirable.
- Experience: 2-3 years of experience in community relations, account management, marketing, or sales is desirable. Home health, hospice or healthcare experience and knowledge required r.
- Demonstrates the ability to communicate both in writing and verbally through both active listening and formal discussions in order to influence key stakeholders at all levels of an organization.
- Exceptional collaborative and relationship building skills required.
- Proficient in Microsoft Word, Excel, PowerPoint required.
- Ability to track, trend and analyze large amounts of data and create reports displaying relevant data.
- Strong problem solving capabilities.
