Unit Details: Prior home health care experience preferred. Strong clinical experience and communication skills. Mileage is reimbursed (visit to visit, office to visit and visit to office only. Workers will not be paid to and from home unless they are reporting directly to a visit from their home.)

Provides nursing service for persons who are home bound and physically unable to visit a physician’s office or other treatment facility. This includes elderly adults with long-term illness or disability, and also persons who are recuperating from major surgery and/ or chronic medical conditions as well as Hospice patients. As a member of the HFM Home Care and Hospice team, the visit nurse will be responsible for the daily delivery of direct and indirect patient care, coordinating follow-up appointments and communicating with the Home Care and Hospice Case Managers.
Accurately and thoroughly conducts home visit as assigned, assessing the patient’s condition, progress toward goals and providing patient care, teaching and other skilled interventions as ordered. Effectively initiates collaboration with other health team members in the care of the patient and family. Contributes to IDT and Episode Management as needed. Consistently documents pertinent information accurately, timely, and completely, Select and administers treatment strategies based upon current physician orders and established plan of care. Documentation reflects skilled interventions that support medical necessity. Utilizes department and JC standards of care as well as the Wisconsin Nurse Practice Act in the delivery of care. Actively and enthusiastically participates in unit-based nursing councils and QAPI teams. Achieves productivity per department standards. Demonstrates critical thinking skills. Effectively collaborates and supports plan of care as determined by the physician and case manager. Notifies patient’s physician and case manager of significant changes in a timely manner. Demonstrates excellent clinical skills with the ability to set up and monitor IVs, administer injections, give tube feedings and perform complex wound care routinely.
One year experience in med/surg and/or home care or hospice nursing preferred. Excellent customer service, communication, and organizational skills required. Must maintain current driver's license and personal auto liability insurance.
Graduate of an accredited school of nursing required, and current unrestricted Wisconsin RN license required. CPR certification required.
