Reports to: Nurse Manager

Compensation: $33.10-$43.67 per hour, plus $1.75 per hour Shift Differential Premium for hours worked between 6pm-6am

Schedule: Monday-Friday 10:00am-7:00pm

Status: 100%, Full-Time, Non-Exempt, Union

Probation: 90 days

DEPARTMENT SUMMARY: As the nation’s largest and most experienced provider of LGBT medical and mental health services, we help LGBTQ people lead healthier, happier lives, by keeping their minds and body well. All services are free or low cost. We’re also one of the few Federally Qualified Health Centers in the nation with providers who specialize in primary care for LGBTQ people and with a research team working to advance the care and treatment of people in our community.

JOB SUMMARY: The LVN will provide routine vocational nursing services and direct patient care with a focus on medication administration for the Sexual Health & Education Program (SHEP). This includes collaboration with providers and other departments, and provision of quality patient care in compliance with local, state and federal regulations and accreditation standards.

ESSENTIAL FUNCTIONS:

• Within the scope of license, provide skilled nursing care, administer medications to patients, and provide patient education regarding medications, diagnosis, and prevention with specific regards to STIs.

• Observe patients for any medication side effects or reactions; document side effects or reactions and report to Medical Provider and Nursing Management.

• Provide patient referrals, books follow-up appointments when necessary, and assist SHEP Provider and Nursing Management when necessary.

• Follow-up on and support treatment and adherence activities.

• Coordinate and communicate with Medical Providers, patients, and pharmacies regarding patient’s medication prescriptions and refills.

• Coordinate and support activities related to prior authorizations and Patient Assistance Programs (PAPs) needed for PrEP and all other medications.

• Maintain complete and accurate files documenting all care, services provided, teaching, and medication administration appropriately in the patient’s health record.

• Contact and provide the necessary follow-up for all patients who test positive for STIs.

• Document all patients with STIs who miss their treatment appointment; contact the patient to reschedule appointment and maintain a call record.

• Assure that examination rooms and treatment rooms are always properly stocked with equipment, supplies, disposable exam materials, etc. and ensure that each exam room is cleaned and set up in the appropriate way.

• Maintain clinical information for all medication treatment and adherence.

• Collaborate with medical and other healthcare providers to plan, organize, and direct treatment and adherence activities including referral to other medical and mental health services.

• Maintain clear communication with team members (i.e., medical provider, nursing staff, case managers, lab staff…) regarding treatment and adherence issues.

• Assist in the Lab and perform phlebotomy, throat swabs, specimen collections, and processing as needed.

• Assist with continuous improvement of total quality care as it relates to nursing services and medication treatment and adherence.

• Monitor clinic refrigerators and maintain temperature logs; notifies supervisor of any out of range temperatures.

• Interact in a professional manner with health services staff and patients.

• Exercise appropriate judgment and decision-making skills.

• Use standard office machinery including personal computers and printers in performance of duties.

• Assist in data auditing tasks, prepare reports as required and submit in a timely manner.

• Participate in staff meetings, agency meetings, planning meetings, and other meetings as needed.

• Comply with requirements of OSHA, HIPAA, and other applicable regulations.

• Other duties as assigned.

JOB QUALIFICATIONS AND EXPERIENCE:

• Knowledge of or experience working with the LGBTQ community and familiarity with issues of particular relevance to LGBTQ people.

• A passion for the Center’s work and its mission to make the world a better place for LGBTQ people.

• Current California LVN License.

• Current AHA CPR certification.

• Minimum of 3 years experience in direct patient care; preferably in an ambulatory care setting.

• Experience in STI and HIV/AIDS healthcare and treatment/adherence experience, preferred.

• Basic knowledge of infectious diseases and state/federal reporting requirements.

• Excellent verbal and written communication skills, including strong organizational, detail, and interpersonal skills.

• Excellent computer skills and knowledge of Microsoft Office, word processing, Electronic Health Records, and other software systems.

• Ability to be self-motivated and dedicated to initiating and completing complex tasks.

• Ability to handle multiple tasks, be detail oriented, and maintain strict confidentiality of medical information.

• Able to work well independently with minimum supervision.

• Bilingual skills, a plus (English/Spanish fluency preferred).

• Demonstrated ability to work effectively with people of diverse races, ethnicities, nationalities, sexual orientations, gender identities, gender expression, socio-economic backgrounds, religions, ages, English-speaking abilities, immigration status, and physical abilities in a multicultural environment.
