ESSENTIAL JOB FUNCTIONS:

Educator:

  • Responsible for development and implementation of a comprehensive patient and family education program.
  • Serve as nurse educator to patients (individually and in group settings), to educate them about a variety of medical, substance use, and mental health conditions, as well as inform them about their recommended treatment.
  • Identify learning needs of patients, as well as their family members when needed, and provide individualized education based upon specific learning needs and preferences.
  • Facilitate learning by gaining an understanding of the patient’s and family’s ability to learn by including the following: cultural and religious practices, emotional barriers, desire and motivation to learn, physical and cognitive limitations, language barriers, and learning readiness.
  • Translate scientific/medical language and concepts into clear, simple, and understandable information for patients, maximizing the potential for them to take reasonable action.
  • Evaluate the effectiveness of the educational intervention and adjust it accordingly, to improve quality results.
  • Utilize an empowerment model to create partnerships with patients and family members by employing strategies based on their goals and preferences to foster self-management.
  • Maximize care skills by increasing the patient and family’s ability to cope with the patient’s health status, prognosis, and outcome.
  • Incorporate knowledge of current clinical practices and technologies to enhance patient’s and family’s ability to be actively involved in care and care decisions.
  • Enhance the patient’s and family’s role by promoting healthy lifestyles.
  • Use computer technology and other innovative methodologies to support patient and family learning efforts.
  • Communicate teaching/learning activities, particularly difficult learning situations and high-risk needs, and patient/family response.
  • Employ clinical practices that are evidence-based.

Health Homes:

  • Oversee the Health Homes Program, including working with patients in the program, reporting, and any other duties required by the program.
  • Serve as the primary contact for Health Homes audits and participate in other audits as needed.

AMAP:

  • Ensure safe utilization of AMAP services in residential and outpatient services by providing education, training, and audits as an Authorized Registered Professional Nurse.

PCMH:

Nurses have a key role in Patient Centered Medical Home (PCMH) and care of patients with chronic medical conditions, by performing the following duties (including, but not limited to):

  • Triage patients and their calls.
  • Provide patient education and clinical advice at the direction of the provider
  • Assist in managing and carrying out patient plan of care.
  • Administer injections.
  • Manage patient rooms- take vital signs, document chief complaint and history of present illness (HPI), and review and reconcile medications.
  • Assist provider in medication refills.
  • Communicate test results to patients.
  • Assist in identifying preventative care needs.

General Nursing Duties:

  • General nursing duties, as needed.

MINIMUM QUALIFICATIONS:

Graduation from an accredited school of nursing and registered with the State Board of Examiners for Registered Nurses. One year as a professional nurse required, with one year of successful experience as a nurse educator, preferred.

KNOWLEDGE, SKILLS and OTHER ABILITIES:

  • Ability to enter information electronically using the agency’s electronic health record.
  • Knowledge of educational principles as they relate to patient and family education.
  • Knowledge of current clinical practices/technologies related to self-care and willingness to learn technologies.
  • Excellent written and communication skills.
  • Knowledge and skills in quality improvement methodologies.
