Seasoned Recruitment is currently seeking a dedicated and experienced Advanced Practitioner (NP or PA) to join our team as a Primary Care provider for individuals with Serious Mental Illness (SMI).

As an SMI Primary Care Advanced Practitioner, you will provide comprehensive primary care services to individuals with SMI, addressing their mental and physical health needs. You will work closely with a multidisciplinary team to develop and implement integrated care plans.

Responsibilities:

• Conduct comprehensive health assessments, including mental health evaluations

• Provide health education and counseling, promoting self-care and illness management skills
