We're hiring for a Community Health Worker (CHW) for the Los Angeles County area! This is a full-time, non-exempt position, offering a competitive salary range, depending on skills and experience. The Community Health Worker (CHW) is responsible for helping members and their families to navigate and access community services, other resources, and adopt healthy behaviors. The CHW supports providers and the Case Managers through an integrated approach to care management and community outreach. As a priority, activities will promote, maintain, and improve the health of members and their family. CHW provides social support and informal counseling, advocates for individuals and community health needs.

Essential Job Functions include the following. Other duties may be assigned.

  • Model, promote and live out the LifeSTEPS workplace values of integrity, innovation, empowerment, respect and personal growth in tandem with the Oz Principles.
  • Follow all LifeSTEPS policies, procedures and guidelines.
  • Responsible for establishing trusting relationships with members and their families while providing support, encouragement and motivation to members.
  • Provide ongoing follow-up, basic motivational interviewing and goal setting with members /families.
  • Follow up with members via phone calls, home visits and visits to other settings where members can be found.
  • Help members set personal health related goals and attend appointments.
  • Provide referrals for services to community agencies as appropriate.
  • Help members connect with transportation resources and provide appointment reminders in special circumstances.
  • Work closely with medical providers to help ensure members have comprehensive and coordinated care plans.
  • Work collaboratively with other clinical personnel assigned to the same member.
  • Knowledgeable about community resources appropriate to needs of members /families.
  • Responsible for providing consistent communication to the Project Supervisor to evaluate member/family status, ensuring that provided information, and reports clearly describe progress.
  • Act as a member advocate and liaison between the member/family and community service agencies.
  • Record member care management information in the Sprite Database and other databases no later than 24 hours after member contact.
  • Attend regular staff meetings, trainings and other meetings, as requested.
  • Manage assigned caseload of members.
  • Maintain HIPPA compliance at all times.
  • Perform other tasks as assigned.

Qualifications


Education and/or Experience and/or License Certification:

  • High school graduate or equivalent required;
  • Bachelor’s degree in social work, psychology or related field.
  • Two (2) years of experience in a similar environment providing social services.
  • An equivalent combination of education and experience may substitute or the degree.
  • Medical terminology and/or background preferred.
  • Written and oral fluency in English and Spanish is preferred.
  • Experience working in a multi-cultural setting.
  • Experience working in a community-based setting for at least 1-2 years preferred.
  • Basic computer skills required, electronical medical record (EMR) experience preferred.
  • Understand the community served, community connectedness.
