**Description**

**$1000 Sign on Bonus**

**Socialization/Leisure/Recreation**

Create opportunities to meet others, engage in social interactions, and develop friendships. Supporting participation in fun social outings and creative activities allows our consumers to pursue their interests, experience new things, and be more involved in their communities.

* Seek out opportunities for community/social integration that enables new and exciting experiences

* Help consumers plan and prepare their choice of fun events and outings

* Create entertaining individual and group activities

**Consumer Advocacy**

* Assist/teach skills that create independence and involvement in all aspects of life

* Utilize consumer strengths to build new skills, achieve goals, and enhance quality of independent living

* Suggest changes to improve consumer lifestyle

**Medication/Health**

* Accompany consumers to scheduled medical appointments and communicate order changes in medication, treatment orders, and therapies

* Assist with medication administration via pill, liquid, injection, rectal, vaginal, inhaled vaporously, through a stoma, or feeding tube

* Provide basic first aid such as controlling bleeding, caring for shock, burns, muscle, bone, and joint injuries, follow poison control procedures, and contact EMS (911) as necessary

* Report changes in consumers medical condition or eating habits immediately to supervisor

* Follow prescribed special dietary programs

* Implement procedures for infection control in compliance with Infection Control Policy and Exposure Control Plan

* Exercise sanitary food service, housekeeping, and laundry practices

* Perform other duties as assigned

**Documentation/Communication**

* Use strict confidentiality practices at all times

* Follow consumers service plan and behavioral support plan

* Maintain complete and accurate records of medical documentation, consumers progress, change in condition, and activities via r-tasks or paper documentation during each shift

* Maintain consumer /site receipts and ledgers, and complete shift audits

* Provide guidance, assistance, or training to others where appropriate

* Interact with others in a positive, cooperative, and effective manner

* Advocate for the consumer by attending requested staffing/meetings and making recommendations for enhancements of the consumers lifestyle

* Communicate consumer needs, health-status change, or concerns to appropriate management staff immediately

* Maintain professionalism when responding to crisis situations and challenging behaviors

* Perform other duties as assigned

**Safe Transportation**

We encourage our consumers to be as active as possible in the community; therefore, safe transportation is a vital part of the services we offer. Only licensed and qualified staff will transport individuals, and are expected to comply with all traffic laws and company policies during operation.

* Conduct a 360 degree walk-around to identify safety concerns prior to operating vehicle

* G.O.A.L. Get Out and Look: Assure there are no hazards in the way

* Motor Vehicle Condition

Check brakes, blinkers, tire pressure, fuel level, oil and other fluids

Report all vehicle incidents and problems with company vehicles immediately

* Use a spotter when backing vehicle when possible

* Utilize back up cameras to ensure safety

**Site Tasks and Safety Practices**

* Assist with meal planning, grocery shopping, and preparation

* Disinfect surfaces and equipment throughout home

* Maintain cleanliness through mopping, sweeping, vacuuming, washing windows, dusting, cleaning the refrigerator, and consistently sanitizing bathrooms and kitchen

* Dispose of waste promptly

* Move soiled items from consumers room to laundry room following proper laundry procedure

* Washing, drying, and replacing bedding, towels, clothing, etc.

* Practice S.A.F.E (stop and focus entirely) strategies. Give your complete attention to the task at hand

* Conduct monthly fire drills and smoke detection checks, and severe weather drills every March and October

* Maintain safe walkways with shoveling and salting/sanding, and perform yard care

* Report safety risks, accident/illness procedures, as well as any contact with emergency medical, fire, and law enforcement agencies to on-call staff immediately

* Perform other duties as assigned

**Assisting with Self Care**

* Dressing and undressing

* Head-to-toe bathing or showering, applying lotion to skin

* Scheduled toileting, incontinence care and clean up

* Assisting with G-tubes, catheters, and colostomy bags

* Brushing/styling hair, applying makeup

* Brushing teeth/denture care

* Shaving

* Clipping nails

* Transferring and repositioning with gait belts, Hoyer lifts, sit-to-stands, other devices/adaptive equipment

* Perform other duties as assigned
