Description: Job Summary:Provides timely and efficient general transport of patients between departments and acts as a discharge escort. Ensures that proper body mechanics, patient mobilization techniques, and patient care protocols are observed during patient lifts, transfers, and transports. Acts as a SAFE Team resource to provide training, guidance and consultation to members of the healthcare team. Maintains a commitment to patient and employee safety as a basic premise of all tasks performed.Essential Responsibilities:Major Responsibilities: In collaboration with the healthcare team, provides or assists with patient care delivery through the essential functions of Mobility Team. At all times works as a team member with the RN and other healthcare team members to provide a comfortable, healing environment for the patient following the principles of responsibility, confidentiality and compliance expectations of all employees to provide safe, quality care. Essential Functions include: Safely transport patients via appropriate method, i.e., wheelchair, stretcher, bed or escorting ambulatory patients, between departments and on discharging of patients to vehicle. Has a thorough working knowledge of all transportation procedures, equipment and safety procedures involved in transporting patients. Has a thorough working knowledge of all lift equipment and techniques to provide safe patient handling. Responds to all Emergency Codes. Prioritizes tasks & demonstrates flexibility as situations occur. Responsible for portable oxygen tank rounds maintain a checklist of tanks checked and replaced. Reports observable changes in patients condition to the appropriate nursing staff. Utilizes patient handling equipment, transfer devices, and protective equipment in compliance with hospital policies, vendor specifications, and patient handling protocols. Assists in safe lifting, transferring, ambulating, transporting, turning and repositioning of patients as requested by the nursing/patient care services team. Utilizes proper body mechanics, identifies appropriate mode of lifting equipment & techniques, and ensures patient care protocols are observed during all patient lifts or transfers and during transport. Maintain, clean and store inventory of wheelchairs, stretchers, beds and lift equipment and other department accessories. Maintains an accurate transport log on the units and on the office data summary log of all transportation activity performed. Performs maintenance checks on all portable lift equipment on a weekly basis. Works cooperatively as part of the nursing/patient care services team and adhere to department and hospital policies and procedures. Demonstrates appropriate judgment in all aspects of patient care. Maintains patient and employee safety as a basic premise of all tasks performed. Picks up, sorts, & delivers clean and dirty special laundry items. Stocks unit lift kits on each unit. Accountable for retrieving and delivering code carts from units to CSP and to units as needed. Transports bodies to morgue. Performs hazard analysis, incident analysis, safety conversations and safety communication as needed. Acts as a member of the hospital decontamination team. Performs Emergency Management duties as assigned, including evacuation as needed. Delivers, returns, and stores specialty beds and safe patient ancillary equipment as needed. Performs other related duties or CNA activities as requested or assigned including but not limited to: obtaining, recording and reporting vital signs, weights, and measuring and recording inputs/outputs. Under the direction of the Nurse Manager or designee, may act as a sitter to ensure that patient safety needs are met and will assist the RN assigned to the patient in providing care. Basic Qualifications: Experience Six (6) months of work experience in a healthcare setting. Education High School Diploma or General Education Development (GED) required. License, Certification, Registration Nursing Assistant Certificate - Level 2 (Oregon) Basic Life Support Additional Requirements:Completed Certified Nursing Assistant II program approved by the Oregon State Board of Nursing (copy of certificate required).Computer keyboarding skills.Ability to speak, write, and read English.Ability to understand and carry out directions or instructions, both written and verbal.Ability to record messages and keep simple records.Ability to accept direction and supervision.Ability to complete and pass Physical Capacity Testing at time of hire. Preferred Qualifications: One (1) year of member of transport/lift team.One (1) year of department/unit Safety Representative/Champion.Previous experience in hospital setting.Previous patient transport and/or lift team experience.Previous patient transport and/or Lift Team experience.Knowledge of patient mobilization and lift equipment skills (Safe Patient Mobility Training Class).Critical thinking skills to ensure the safe transportation and mobilization of patients.Knowledge of KPNW workplace safety programs.
