Position Overview:

Ignite your career with passion and purpose with a remote tele psych, full-time or part-time employee or contract basis as a Licensed Marriage and Family Therapist (LMFT) at JWCH! Immerse yourself in the heart of Southern California, covering one or more of our Primary Care Clinics, Residential and Out-Patient Substance Use Programs.

As a full-time Licensed Marriage and Family Therapist, you'll play a vital role in providing behavioral health services to our diverse patient population, including individuals experiencing homelessness. Based at one of our sixteen Primary Care Clinics in Southern California, you'll deliver compassionate care, crisis intervention, and counseling services to those in need. Your work will directly impact the health and well-being of our patients, ensuring they receive the support and resources necessary for positive outcomes.

Key Responsibilities:

  1. Work within a Federally Qualified Health Center multidisciplinary team to ensure that patients' clinical treatment goals and behavioral management needs are met by attending case conferences, chronic disease management meetings, staff consultations, and individual meetings with staff to ensure quality of care.
  2. Collaborate within a multidisciplinary team to develop and implement comprehensive treatment plans.
  3. Conduct assessments and diagnoses for mental health, substance abuse, trauma, and psychosocial needs.
  4. Provide in-person and remote (via HIPAA Compliant software) psychotherapy, counseling, medical case management, and psycho-education utilizing individual, family, and group modalities using evidence-based interventions for patients and their significant others. This includes social, emotional, and practical life skills to facilitate safe coping, distress tolerance, stress reduction, mindfulness, relaxation, and access to necessary resources with the goal of improved functioning and positive health outcomes.
  5. Utilize short-term interventions, as approved by the funding source, targeting trauma, mental health symptoms, increasing psychosocial functioning, and reducing substance use.
  6. Consult with primary care providers to enhance understanding of the patient, provide support for treatment approaches incorporating behavioral health needs, and assist with implementation of bio-psychosocial treatment and care plans.
  7. Facilitate linkages to social services and housing resources.
  8. Participate in community meetings and Multi-Disciplinary Team meetings.
  9. Offer after-hours emergency psychiatric assistance and phone consultations as needed.
  10. Complete all required documentation in compliance with organizational standards.

Requirements:

  1. Master’s degree in Psychology or equivalent.
  2. LMFT with a current California BBS license.
  3. Demonstrated experience/knowledge in human, social, and/or behavioral health services, particularly with low-income underserved individuals.
  4. Knowledge of trauma-informed care, person-centered care, and evidence-based treatments.
  5. Knowledgeable about evidence-based treatments such as DBT, CBT, Seeking Safety, and Motivational Interviewing.
  6. Familiarity with mental health emergency procedures and reporting protocols.
  7. Preferred experience in substance abuse disorders and medical settings.
  8. Bilingual (English/Spanish) ability strongly preferred.
