Mission Statement:

The Mission of JWCH Institute is to improve the health status and well-being of under-served segments of the population of Los Angeles Area through the direct provision or coordination of health care, health education, services, and research. The mission is being accomplished through a variety of programs and activities, such as: medical outreach and referrals for medical care, HIV services and drug treatment; health education; psychosocial assessment and intervention; primary medical care; family planning services; and research.

Program: Ending the HIV Epidemic Intervention Program-Cisgender Women with HIV

Position Purpose:

Under the direction of the Program Manager, the Community Health Worker will play a key role in implementing and achieving the objectives of the Ending the HIV Epidemic Intervention Program for Black/African American and Latinx Cisgender Women with HIV. This involves conducting outreach and presentations to promote the program, recruiting participants, facilitating group sessions, offering support in referring participants to medical and support services within the agency, and contributing to data collection and program reporting.

Principal Responsibilities:

  1. Actively seek opportunities for outreach and presentations, focusing on identifying hard-to-reach populations and effectively promote awareness of the program.
  2. Perform outreach activities to recruit and enroll clients in the EHE Intervention program.
  3. Develop rapport and establish trusting relationships with clients during Seeking Safety sessions and follow-up to reinforce a positive lifestyle.
  4. Engage with the homeless population to promote participation in the ongoing reinforcement of learned skills by program participants.
  5. Assist in the facilitation of group sessions to create a supportive and informative environment.
  6. Providing valuable support in referring participants to essential medical and support services within the agency.
  7. Assist in gathering and compiling program data for the Project Coordinator/Manager and ASW.
  8. Provide support and help participants build emotional and social support systems to motivate them to keep scheduled appointments, retain in care, and achieve viral load suppression.
  9. Participate in JWCH, DPH, and other scheduled trainings.
  10. Perform other duties as needed.

Requirements:

  1. Strong ability to develop positive relationships with clients living in temporary housing (e.g., shelter, mission, hotel, or substance abuse recovery facility).
  2. Familiarity or experience working on HIV intervention programs or similar initiatives.
  3. Experience in facilitating group sessions and engaging with diverse populations.
  4. Excellent verbal communication skills.
  5. Ability to work with homeless, mentally ill, and/or substance-abusing adults, as well as professional staff.
  6. Access to transportation to and from the office and all program-related sites.
  7. Sensitivity and understanding of the cultural nuances and healthcare disparities within Black/African American and Latinx communities.
  8. Reliability in the performance of job duties; timely arrival for outreach and all sessions.

Preferred:

  1. Previous experience in community outreach and health promotion.
  2. Familiarity and/or experience with Seeking Safety Intervention.
  3. Experience working with the homeless population.
  4. Spanish-speaking and/or conversant.

JWCH Institute, Inc., offers competitive salaries for all positions. Full-time employees working at least 30 hours per week are eligible for medical, dental, vision, and life insurance. Full Time Employees are also provided a monthly allowance which can be used towards insurance premiums.
