*Make a Difference in End-of-Life Care:*
As a skilled and compassionate RN Case Manager, you'll play a vital role in supporting terminally ill patients and their families during a challenging time. You'll provide expert care coordination, education, and emotional support, ensuring a dignified and comfortable experience for all involved.
*What You'll Do:*
* Conduct patient assessments and coordinate care plans with the nursing team, aligning with patient needs and family wishes.
* Educate patients, families, and caregivers on medication management, medical equipment usage, disease progression, and hospice philosophy.
* Address patient and family concerns with empathy and clear communication, providing exceptional care and emotional support.
* Demonstrate a deep understanding of hospice principles, pain management, and the unique needs of the terminally ill.
* Collaborate effectively with other healthcare professionals to ensure seamless care delivery.

*We're looking for a dedicated RN with:*
* Current and active Texas RN License
* BLS Certification (required)
* Minimum 2 years of experience in hospice or palliative care (preferred)
* Strong clinical assessment and care coordination skills
* Excellent communication and interpersonal skills, with a focus on empathy and active listening
* Ability to educate patients and families in a clear and compassionate manner
* Commitment to compassionate, holistic care for terminally ill patients
