Job Description: We are seeking an energetic, patient, thoughtful, and outgoing Community Facilitator to support & assist multiple individuals in the surrounding community primarily in the Santa Clarita and San Fernando Valleys. You will provide on the job coaching for people who are employed, as well as support in the community for people to address goals around community safety & awareness, social interactions & self-help skills. Responsibilities:
Provide one-to-one support to an individuals at work, in their home & community who have a diagnosis of Autism or other developmental disabilities & has limited safety awareness.
Implement the individuals Support Plan as developed
Provide the necessary accommodations to support an individual with various challenges (behavioral, physical, communication, etc.) within their home and community
Support the individual to establish a daily/weekly schedule of activities.
Act as a mentor and role model for socially acceptable and culturally valued ways to interact and support each other to be successful
Participate hands-on in all activities
Assist and support the person to maintain their own personal safety and well-being
Provide information to family members about the unique needs of the individual to assist in the building of a more cohesive family unit.
Comply with all policies and procedures of the organization and public locations of activities

Qualifications
Experience supporting individuals with Autism and or developmental disabilities preferred not required.
Energetic, outgoing, patient, and respectful
Have a high school diploma or GED
A valid drivers license, reliable auto & current auto insurance ( Case by Case basis)
Ability to handle physical aspects of job
Ability to interact effectively at all levels and across diverse cultures
Comply with all policies and procedures of the organization and public locations of activities
