This position has a residency waiver and will be accepting applications from residents and non-residents of Colorado. About Fort Logan:
The Colorado Mental Health Hospital in Fort Logan (CMHHIFL) is one of two state hospitals serving our community. The vision of CMHHIFL is to provide the highest quality mental health services to persons with complex, serious, and persistent mental illness within the resources available. Our mission is to support those individuals in achieving their recovery goals and reaching their full potential. CMHHIFL provides inpatient treatment to adult patients, generally between the ages of 18-59. The hospital currently has 138 adult inpatient beds and over six treatment milieus serving civil and forensic individuals. Medical services, occupational therapy, neuropsychological-rehabilitation services, and trauma-informed care services are available for all patients throughout the hospital.

Description of Job
The positions filled by this recruitment could be staffed within the Civil and/or Forensic treatment units.
Summary of Position:
The Social Work II position works in the 24-hour State psychiatric hospital with individuals voluntarily and involuntarily committed for treatment. Patients receiving treatment at CMHHIFL have severe and persistent mental illness and may have traumatic brain injury, trauma disorders, and other complex dynamics. Each treatment team is multidisciplinary, including psychiatrists, psychologists, social workers, addiction specialists, occupational therapists, recreation therapists, creative arts therapists, nurses, physicians, and mental health clinicians. In addition, the hospital offers dietary, peer, and chaplain services
The Social Work unit collaborates with patients, their families and/or support systems, and external stakeholders to plan, organize, and execute psychiatric treatment interventions and comprehensive aftercare plans with placement. Services include clinical assessment, treatment planning, individual therapy, group therapy, family therapy, counseling, and education. This work unit serves as a liaison between the hospital Community Mental Health Centers (CMHC's) and other governmental entities.
The purpose of the Social Worker is to provide individual therapy, group therapy, and family therapy under clinical supervision . Individual and family therapy is provided as needed, which is determined in the treatment plan meetings and in collaboration with the treatment team. Group therapy is offered to all patients and group assignment shall be determined by clinical and individualized needs. The Social Work positions are responsible for developing group materials appropriate for patient-specific needs and leading group therapy sessions. The Social Worker completes comprehensive biopsychosocial assessments and reassessments, coordinates treatment planning for all patients, and regularly documents in the electronic health record. The Social work team is responsible for the coordination of discharge and comprehensive aftercare plans. In doing so, this position collaborates with support systems, stakeholders, CMHC's, and other agencies to coordinate continuity of care. The Social Worker actively contributes to hospital-wide events, in-service training, and program development.
Social Worker II supports the execution of the hospital's mission, vision, and values within the social work department and in collaboration with leadership. The Social Worker II provides supervised clinical service provision by ensuring their work product contributions are reviewed, complete, and timely. This includes treatment plans, documentation, assessments, and therapeutic interventions.
Case Management and Discharge Planning:
  • This position must assess and decide client needs for financial, personal, level-of-care, and community support and then facilitate that linkage.
  • This position must independently determine and ensure tasks are completed on behalf of the patients assigned to their caseload and ask for support when appropriate.
  • The social worker identifies potential discharge options and the necessary resources/benefits to support the discharge placement.
Assessment, Intervention, and Treatment Planning:
  • The Social Work II position regularly serves as the liaison between the individual in care at CMHHIFL and their support system, as well as community stakeholders. As such, they are responsible for determining what information can be shared based on various laws and court orders.
  • Responsible for determining which treatment interventions are indicated and determining what community support may be needed to support continued intervention post-discharge.
  • Leads the team in determining which short-term and long-term treatment goals meet SMART goal criteria and should be included in the individual treatment plan.

Minimum Qualifications, Substitutions, Conditions of Employment & Appeal Rights

Minimum Qualifications:
Master's Degree in Social Work
Current, valid licensure as a Social Worker (LSW) from the Colorado State Board of Social Work Examiners.
AND
One year of experience as a Social Worker.
Substitutions:

Special Qualification:
Licensed Clinical Social Worker (LCSW)
Preferred Qualifications:
  • Knowledge of DSM 5-TR
  • Prior adult inpatient experience
  • Desire to work with adults who have been diagnosed with severe and persistent mental illness
  • Experience and/or desire to work with diverse, under-served client populations.
Conditions of Employment:
The successful candidate in this position must be willing and available to submit to the following conditions:
  • Full Background: Name check, TRAILS, CAPS, Drug Screen and Fingerprinting
  • Must possess a valid, unrestricted U.S. Driver's License
  • Incumbents of this position must be at least 21 years of age.
  • Must be able to use, handle, or be exposed to hazardous material, such as bodily fluids.
  • May be able to travel to attend training, court hearings, transport patients to appointments, and other related activities.
  • Current valid, licensure as a Social Worker (LSW)
  • Former State employees who were disciplinarily terminated or resigned in lieu of termination must disclose the information on the application and provide an explanation why the prior termination or resignation should not disqualify the applicant from the current position. Absent extraordinary circumstances, prior disciplinary termination or resignation in lieu of termination will disqualify the applicant from future State employment with CDHS.
