JOB SUMMARY |
Under the general supervision of the Program Director/designee, functions as part of a team assigned to support people living in independent apartments in the community who have a history of addiction. Substance Abuse Case Managers provide specialized support to people suffering from or suspected of substance use disorder. They assist assigned individuals in developing and maintaining viable living, working and social situations in the community by obtaining needed medical, social, psychosocial, educational, financial, vocational, and other services. They participate in the provision of crisis intervention services to clients and travel to/visit clients' residences or apartments to provide counsel and assistance and to help them within their capabilities and interests, in maintaining the greatest degree of independent living. |
ESSENTIAL JOB FUNCTIONS: List all essential job duties. § Provides Intake and initial assessment of client's entering the Supported Housing Program; § Responsible for weekly Client contact, monthly homes and hospital visits as needed; § Designs and implements service plans for all assigned clients; § Provides medical monitoring and oversees medication compliance – during home visits and/or case conference with medical providers; § Oversees mental health and/or Methadone treatment program compliance and or referrals as needed; § Provides crisis Intervention as needed, conferencing with health care providers for a referral when needed; § Assists Entitlements staff in the securing of entitlement benefits, and other appropriate service delivery via internal or external referrals for services; § Uses team approach while conducting case review and/or chart review to ensure client service and contractual compliance; § Prepares narrative and statistical reports on a regular basis as assigned; § Participates in case conferences and networking, both internal and external, linkage development; § Provides on-going support and case management services to individuals on assigned caseload ; § Maintains a strengths based and person centered approach to treatment; § Utilizes Motivational Interviewing and other Evidenced Based Practices; § Adheres to a Harm Reduction Approach to practice; § Coordinates with Health Homes, Therapists and other community providers in order to reduce relapse and in-pt. hospital stays; § Works with clients to develop individualized case management plans; § Ensures that all client case management plans address identified needs and are up-to- date; § Continually monitors and evaluates each client’s progression through their case management plan, and develop corrective action revisions to the plan as needed; § Maintains accurate documentation of service objectives and outcomes as well as other services in accordance with Federal, State, County and ICL guidelines; § Assists clients in obtaining necessary resources to achieve case management goals; § Actively participates in client- and program-related meetings and trainings; § Attends program-related community, coalition and committee meetings as assigned; § Maintains client related data systems, including case notes and entries using prescribed agency software programs; § Prepares case management related reports including but not limited to: outcomes, successes, etc; § Adheres to strict boundaries and professional ethics in the care of others; § Conducts crisis intervention as needed. § Performs other duties as may be assigned |
ESSENTIAL KNOWLEDGE, SKILLS AND ABILITIES |
· Ability to effectively use required software such as Anasazi, Word, Outlook, and other technology required by ICL. · Basic understanding of the causes and processes of mental illness and substance abuse disorder. · Willingness to continually learn and apply knowledge and willingness to participate in in-service training and development activities. · Ability to function as an effective team member, including performing share of work, cooperating with coworkers, and securing cooperation, and maintaining professional relationships and boundaries with co-workers, supervisors and consumers. · Knowledge of addiction/recovery processes along with related resources and the Continuum of Care for homeless persons; · Ability to work with consumers/residents, families, and staff in a caring and respectful manner, and with due understanding of and consideration for cultural differences. · Ability to serve as a role model to residents/consumers, including modeling appropriate interpersonal interactions, demeanor, etc. |
QUALIFICATIONS AND EXPERIENCE |
Minimum: Bachelor’s degree plus two years’ experience providing direct services to individuals with mental disabilities , substance use disorder or other relevant human service experience, or bachelor’s degree that required a one-year practicum working directly with individuals with mental disabilities (e.g., BSW) and one year experience. Must provide own transportation for work-related travel as necessary Preferred: BA with major concentration or internship in a human services field.
