Outcome Specialist

InReach is looking to hire a Part-Time Outcome Specialist.

We have an exciting opportunity to join our team as an Outcome Specialist! This role has the opportunity to work a hybrid schedule, working mostly remotely but will be spending time at the office located in Charlotte, North Carolina.

Schedule: Tuesday & Thursdays 2:00 pm-7:00 pm; Saturday 10:00 am-3:00 pm (Schedule may change as we build onto the position, this includes increased hours up to 20/week)

Pay Rate: $15.00/hr

This position is partially remote and in-office. The office location is 4530 Park Road Suite 300, Charlotte, NC 28209.

Responsibilities:

• Working directly with a person(s) who has a developmental disability
• Facilitate community integration
• Transport and accompany people to community activities
• Assists with planning, coordinating, and implementing individualized dreams and objectives chosen by the person
• Teaches Person-Centered and Self Directed skills when working with people
• Records the experiences and shares success stories of people supported
• Work closely with the Accreditation Specialist reviewing data collected from interviews of people supported
• Ensures agency and program functions.
• Protects operations.
• Cares for customers.
• Respect customers including rights and support with education and exercising of those rights.
• Facilitates access to community resources.
• Contributes to team effort and supports people in communicating with their support.
• Supports people with advocacy pertaining to service provision, and education of options for services.

Requirements:

• High school graduate or equivalent
• Prior experience in directing activities for individuals with special needs preferred.
• Must have basic computer knowledge with strong Microsoft Office skills.
• Possess a valid driver's license and maintain a safe driving record.
