Illumination Foundation (IF) is a growing non-profit organization dedicated towards disrupting the cycle of homelessness by providing targeted, interdisciplinary services in our recuperative care centers, emergency shelters, housing services and children's and family programs. IF currently has 13 facilities with 22 micro-communities scattered across Orange County, Los Angeles County and the Inland Empire.Job DescriptionThe Outreach Lead Care Manager will review multiple Medi-Cal Managed Care Plan Member Information Files (MIF) and strategize engagement methods to evaluate Members health and managed care needs. This candidate functions as an extension to the Lead Care Manager team by providing a foundation of Client Care through detailed information of health vulnerabilities and disparities during enrollment.The pay range for this position is $22.00 - $24.00 per hour, depending on experience.ResponsibilitiesThe Outreach Lead Care Manager will use a variety of communication modalities, community-resources, rhetorically appropriate approaches, and triage-oriented screening methods to performing outreach & engagement to prospective members acrosssocial determinants of health and in culturally and linguistically competent ways.An Outreach Lead Care Manager will have an expert understanding of how to utilize different communication modalities when trying to reach their assigned Member list.The Outreach Lead Care Manager will be accomplished in the form of time-utilization and will field high amounts of communications and enrollment conversion rates.Outreach Lead Care Manager will be able to use rhetoric effectively when speaking to Members, affiliate parties, care team members, PCPs, etc.Outreach Lead Care Manager will have familiarity with techniques such as trauma-informed care, motivational interviewing, cultural and linguistic competency, and de-escalation.Ability to enroll 5% of assigned caseload.Documentation:Outreach Lead Care Manager will be responsible for appropriately navigating and documenting all engagement frequency and content in Illumination Foundations EMR and other documentation platforms as required by contracting entities, in accordance with standards of practice as required by Illumination Foundation and associative contracting entities.
