Human Touch Home Health is currently hiring a Full-Time Clinical Manager for our location in DMV area.
THIS POSITION REQUIRES UP-TO-DATE NURSING LICENSE IN Washington, DC/Virginia/Maryland
Job Description
Responsible for planning, coordinating, managing, and directing all activities and programs.
Essential Job Functions/Responsibilities:
  • Receives case referrals. Reviews available patient information related to case, including disciplines required, to determine home care needs. Assigns appropriate clinicians to case.
  • Establishes and maintains positive working relationships with current and potential clients.
  • Promptly responds to customer’s request and concerns.
  • Reviews and evaluates each case by reviewing the services provided by clinicians, conferences, record review, discusses and verifies impressions, instructs and guides clinicians to promote more effective performance and delivery of quality home care services, and available at all times during operating hours to assist clinicians as appropriate.
  • Reviews patient’s clinical diagnosis, medications, procedures and clinical course.
  • ICD-10 Coding for patients on case load.
  • Review and process all OASIS for patients on case load.
  • Establish immediate and long-term goals, setting priorities, and develop an individualized plan of care.
  • Updates the Plan of Care as needed to represent the patient’s current needs.
  • Conducts Case Conference Meetings with clinicians to facilitate and coordinate care to assure the Plan of Care is implemented. .
  • Coordinates and oversees all direct and indirect patient services provided by clinical organization personnel.
  • Provides guidance and counseling to coordinators and Clinical Supervisors to assist them in continually improving all aspects of home health care services, provided through organization personnel and more...
Qualifications
  • Bachelor's degree in Business Administration, Masters degree in Health care or related field preferred.
  • RN
  • At least three to five years of experience in healthcare management in health care operations
  • Demonstrated an ability to supervise and direct professional and administrative personnel.
  • Ability to market aggressively and deal tactfully with the community.
  • Knowledge of corporate business management
