Human Services Administration Organization is Hiring a Youth Support Partner Near Pittsburgh, PA

HSAOs JPT program is an empirically based process that empowers the most high-risk youth and families in Allegheny County by building a cohesive team of professionals and natural supports. This team uses monthly meetings to design a personalized plan for families to reach their vision for a better future.

The Youth Support Partner is a full-time position that works with youth throughout Allegheny County who have agreed to participate in the Joint Planning Team Process. The Youth Support Partner will work as part of the Joint Planning Team Workforce, which includes Coach, Facilitator, and Family Support Partner. Youth Support Partner provide support and guidance to the youth throughout the JPT process, during meetings, and in completion of the individualized plans developed by the youth and their family.

The Youth Support Partner will work with up to 25 youth to develop and effectively utilize their own team of supports (natural, community, and systems).

The Youth Support Partner will assist youth in utilizing JPT process, achieving their goals, pursuing healthy life choices, handling adversity, navigating peer pressure, connecting with resources, helping youth have a voice, and be accountable for their future. This position involves connecting with youth while working independently and with teams, in order to identify, advocate and act as a liaison between youth and other professionals and community natural supports.

The Youth Support Partner will use their own life experiences to model for JPT youth how to navigate systems using their voice to meet their needs while developing natural supports within their own community to reach their family vision.

This individual will support and promote child-driven, family focused services to youth and families enrolled Joint Planning Team Program.

The Youth Support Partner will work collaboratively with HSAO staff in the planning and facilitation of Teen Council and Teen Group opportunities.

This position reports to the Joint Planning Team Coach.

DUTIES AND RESPONSIBILITIES

  • Maintain regular contact for up to 25 youth
  • Attend monthly interagency JPT Planning Meetings with assigned youth, encouraging them to speak up about their own strengths, culture and needs
  • Support client progress through the 4 Phases of High-Fidelity Wraparound
  • With the help of the JPT Facilitator, the YSP supports ensures the phases, activities, principles, and theory of change are accomplished with the clients
  • Work with team to gather information needed to complete program documents and surveys
  • Ensure youth understand and agree with their individualized Strengths Needs Culture Discovery, Functional Assessment, Crisis Plan, Action Plans, Transition Plan, etc.
  • Meet with the team and provide excellent communication regarding the familys story, culture, strengths, and concerns/ needs to contribute to the effective HFW process
  • Support and encourage youth to increase healthy relationships with natural supports
  • Establish respectful relationships with persons served to help those gain skills and confidence
  • Work collaboratively with other personnel and/or service providers.
  • Act as agency representative in activities involving youth voice and input as requested
  • Complies with and adheres to HIPAA confidentiality policy as evidenced by keeping all files locked, not
  • releasing any third-party information, and completing filing weekly
  • Submit case notes / documentation in timely manner that accurately reflects the interactions with clients, families, team, community partners
  • Meet utilization standards set by agency
  • Attends and participates in Professional Development Plans and remains competent and knowledgeable of current trends.
  • Active participant in monthly YFTI Youth Support Partner calls
  • Attend in service and training as required.
  • Identify and link youth to community resources
  • Attend at least 3 hours of group or individual coaching/supervision per week
  • Other duties as assigned

KNOWLEDGE, SKILLS, AND ABILITIES

  • Strong oral and written communication skills.
  • Strong interpersonal skills
  • Ability to manage time by prioritizing and organizing activities to completing duties within deadlines.
  • Demonstrate sensitivity to the cultural and socio-economic differences present among the agencys defined-service population.
  • An outgoing personality that can engage people from different cultures, ages, and backgrounds.

ESSENTIAL REQUIREMENTS

  • Able to obtain Act 33, Act 34 and FBI clearances.
  • Ability to work independently as well as part of a team.
  • Full-time position requiring 38.75 hours per week.
  • Have life experience from which you possess a diverse understanding of the struggles youth with complex emotional needs endure.
  • Have successfully navigated child-serving systems (mental health, child welfare, probation, special education, etc.)
  • Valid drivers license and access to a reliable vehicle to attend meetings and make home visits to the families. Local travel as required.
  • Some evening availability will be required to meet the needs of youth
  • HFW Youth Support Partner credential within 18 months of hire
  • Ability to utilize computers and office equipment to complete daily work responsibilities.

EDUCATION AND EXPERIENCE

  • High School Diploma or equivalent
  • Minimum of 1 year of work experience
  • Must have: (1) personal life experience in one or more of the child serving human service systems: juvenile justice, behavioral/mental health, intellectual disability, child welfare, drug & alcohol, special education, etc, and (2) the ability to positively and appropriately self-disclose regarding that experience, (3) Concluded involvement with HSAO, child welfare, and/or juvenile justice at least 1 year prior to start date
