Interdisciplinary team work to support outstanding patient and family care Position Description: We are seeking a compassionate Registered Nurse (RN) to join our interdisciplinary hospice team in providing high quality, expert care to our seriously ill patients. Our RN Case Manager is a vital part of our hospice team providing comfort, care, support and education to our patients and their loved ones. The Ideal Candidate Will Possess the Following Requirements: Valid, unencumbered Missouri and Kansas RN or compact license to include both states
Minimum of one year experience in caring for hospice patients is preferred
Ability to communicate effectively and professionally with others; and to prepare and present information in oral and written formats
Superior organizational skills with the ability to multitask and meet deadlines; must schedule and manage time effectively
Valid driver’s license and insurance plus reliable transportation
