The Nurse position– will facilitate Hospice of the Western Reserves (HWR’s) efforts to relieve suffering, enhance comfort, promote quality of life and foster choice in the presence of an advanced serious illness for all patients and families. The practice is based on competent clinical knowledge and focuses on clinical practice, education and collaboration related to palliative and hospice care. This position will be based out of our Lorain County office in Lorain, Ohio. The primary hours are 8am-4:30pm, Mon-Fri.

WHAT YOU WILL DO:

Clinical Practice

  • Uses the nursing process to gather information, assess (RN), plan, implement and evaluate patient care to provide for the unique physical and emotional and training needs of each patient with the availability to provide handouts, videos etc.
  • Establishes and maintains communication and collaboration with the interdisciplinary team and patient’s primary physician regarding patient condition and plan of care.
  • Anticipate needs to ensure exceptional patient centered care
  • Completes phone follow up with patients who have called in over the weekend or after hours.
  • Completes phone calls from the care solutions center by fielding incoming nursing-based calls throughout the day
  • Completes phone follow up with patients who have been discharged from the hospital.
  • Phone support includes questioning and noting medication changes, increased care needs and/or changes in condition following hospitalization. Based on this they will either verify the next nurse visit or request sooner a visit.
  • Be able to communicate referral criteria based on physical symptoms, functional/cognitive changes with patients/families and refer to hospice level of care when needed
  • Coordination of care within the agency, between transdisciplinary team members and with external partners
  • Documentation is consistent with HWR’s guidelines. Documentation paints a clear picture of the conversation and outcome of the call
  • Contributes to and follows the plan of care in alignment with patient/family goals, collaboration with primary nurse, social worker (SW), HNAS, SCC and other members of primary team
  • Complies with program practices and guidelines in all interactions with team members, patients, or families.
  • Perform chart review and determine need for sooner visit by primary nurse. Maintain spreadsheet with assigned information
  • Phone calls to patients/families per team’s direction for things like follow up on medication changes, upcoming change in address, training needs etc.
  • May participate in transdisciplinary team conferences. To include offering insight on patients discussed or patients needing to be discussed based on phone support.
  • Participates in the back-up and on-call rotation.
  • Rotates holidays

Education

  • Educate and train patients and families on applicable areas of hospice care, such as interventions for symptom management within the scope of practice.
  • Empowers patients and families to identify goals of care

Collaboration

  • Works as part of a team
  • Collaborates with members of the primary care team
  • Collaborates with health care professionals as appropriate within the community
  • Facilitates communication between nursing staff/physicians and patient/family
  • Maintains and cultivates professional relationships in healthcare that strengthen HWR’s strategic marketing plan

SUCCESS CAPABILITIES:

  • Maintains a current Registered Nurse and or LPN license in the state of Ohio, LPN preferred
  • Minimum of two years’ experience in hospice and palliative care preferred
  • Must be flexible and have problem solving skills, prioritize and demonstrate health coping strategies
  • Ability to demonstrate excellence in written and oral communication.
  • Knowledge of hospice and palliative care clinical standards of practice, policies, and procedures
  • Ability to practice in accordance with HWR clinical standards of practice, policies, and procedures
  • Ability to establish and maintain collaborative working relationships with HWR employees, community, and healthcare providers

CONDITIONS OF EMPLOYMENT:

  • Compliance to Annual Flu Shot Policy or ability to provide exemption documentation
  • Provide an active auto insurance policy as a licensed driver in the State of Ohio with the proper level of coverage as directed
  • Provide an active driver’s license
  • Ability to provide proof of eligibility to work in the United States.
