Job Summary: The Community Outreach Coordinator is responsible for developing, maintaining, and strengthening relationships on the field with new and existing referral sources who can refer clients to our company. He/she is the key member of our Business Development team and helps lead the creation and implementation of referrals that will increase patient census, branding and company awareness.

Responsibilities:

• Community Partnerships: Collaborate with local food pantries, senior centers, faith-based organizations, and other community resources to foster partnerships and expand outreach efforts. Actively participate in community events and initiatives to strengthen relationships and enhance visibility.
• Sales Presentation in group setting: Conduct engaging and informative sales presentations in group settings, including but not limited to community centers, senior centers, and faith-based organizations, to educate audiences about the benefits of home care services and the CDPAP program.
• Street Marketing: Implement creative and effective street marketing strategies to increase awareness of home care services and the CDPAP program within the community. This may include distributing flyers, hosting informational booths, and engaging with community members directly.
• Community Engagement and Promotion: Promote home care services within the community, leveraging language skills to effectively communicate with diverse populations. Develop and nurture new referral sources within the community through targeted outreach efforts.
• Relationship Strengthening: Conduct daily visits to various organizations, physician offices, hospitals, assisted living facilities, and adult day care centers to educate and inform referral sources and patients about available home health services.
• Availability to Close: Demonstrate the ability to effectively close deals and convert leads into referrals for the Consumer Directed Personal Assistance Program (CDPAP).

Event Coordination and Representation: Coordinate and actively participate in health fairs, awareness days, cultural functions, and religious events to raise awareness and establish connections within the community.

Requirements:

• Fluent in English and at least one or more of the following languages: Bangla, Bengali, or Spanish. 1 year of experience in outreach sales or a related field preferred. Must possess reliable transportation and driving skills.
