Patients requiring Physical Therapist support may have experienced accidents or have conditions such as spinal discomfort, arthritis, cardiovascular issues, fractures, head injuries, or paralysis. Responsibilities entail creating treatment plans and utilizing diverse approaches including physical exercises, therapies, manual therapy, and technology to provide quality care

$55 - $75 hourly

• Providing an initial and ongoing assessment to determine the level of functioning, including OASIS assessment at appropriate time points
• A physical therapist gathers background data, reviews referrals, selects and modifies evaluation tools, and evaluates patients determining the level of the patient’s physical and psychosocial functioning
• Developing and revising the plan of care in consultation with the physician and other care team members
• Responsible for planning, evaluation, and treatment implementation of assigned caseload
• Preparing clinical progress notes
• Supervising therapy assistants as appropriate

Education: Currently licensed

Experience: Two years of experience in Home Health is preferred
