Starting Salaries: $57,600 - $73,800 annually LLMSW / LMSW / LPC - offer may be higher based on years of experience.

InterCare is searching for a full-time Outpatient Therapist to join our team at the Holland Health Center! Monday - Friday from 8:00am - 5:00pm

Requirements

The Outpatient Therapist provides integrated behavioral health services to patients currently receiving primary care services. As a licensed clinician, the Psychotherapist will provide CBT (Cognitive Behavioral Therapy) and other EBPs (Evidence Based Practices) to improve patients’ psychosocial functioning and health, as well as Mental Health and SUD disorder treatment services to children, adolescents and adults.

Description of Primary Duties & Responsibilities

  • Provide consultation and assistance to medical providers on the management of behavioral health concerns in primary care and help implement the team-determined treatment plan.
  • Conduct comprehensive screenings to evaluate the needs of adults, adolescents, and children for behavioral health intervention, case management, or ongoing mental health or addiction treatment. Provide referrals for more intensive services where appropriate.
  • Provide behavioral interventions to engage and motivate Patients to actively manage their care. Conduct follow-up as appropriate based on assessment and treatment. Provide crisis management for patients as needed; make linkages for interventions as appropriate.
  • Provide Individual Psychotherapy, Group and Family Psychotherapy as needed.
  • Participate in ongoing training in behavioral health issues and provide consultation and training to primary care clinic staff in mental health and behavioral medicine topics including communication skills, brief interventions, counseling skills, medication use and misuse, substance abuse, wellness, rehabilitation, recovery and self-sufficiency.
  • Establish and maintain a current listing of community linkages to meet the needs of the target population. Develop and maintain collaborative working relationships with community providers. Initiate linkages, disseminate information and make referrals to the appropriate services as needed.
  • Conduct patient satisfaction surveys with patients receiving behavioral health services under the program. Also, participate in program model development and evaluation, and collect and compile data as required by program model leads.
  • Participate in regular multidisciplinary case conferences with medical providers as needed. Complete population health projects as assigned and panel-manage patients in collaboration with other behavioral health staff.
  • Meets adopted provider performance expectations.
  • Meets established peer review outcomes.
  • Meets daily productivity expectations (5 billable encounters per day).
  • Meets documentation standards in the EHR.
  • Meets patient and team satisfaction expectations

General Development

  • Ability to work effectively with a wide variety of people and develop and maintain professional working relationships with site and administrative staff.
  • Bilingual in English/Spanish required.
  • Possesses skills in motivation, self-direction, and punctuality.
  • Clinical experience with provision of mental health services in a community-based setting.
  • Knowledge of health information related to adults, adolescents and/or children.
  • Knowledge of, and experience working in, a primary care facility as a mental/behavioral health specialist preferred.
  • Ability to work well as part of a team and keep team members updated on current project developments.
  • Remains flexible and calm while performing duties in stressful environments. Contributes to a positive and respectful workplace.
  • Maintains professional and technical knowledge by attending educational workshops, reviewing professional publications, establishing personal networks, and participating in professional societies.

Professional and Technical Knowledge

  • Master’s degree in Social Work, Psychology, Counseling or Human services field. Must have LLMSW, LMSW, LPC or LLP (limited licensed psychologist) licensure in Michigan. Must be on CAADC development plan or have CAADC (Certified Advanced Alcohol and Drug Abuse Counselor).
  • Experience with Motivational Interviewing and CBT.
  • Maintains a working knowledge of Joint Commission, PCMH, and IHI care coordination and integration standards.
  • Understands and consistently supports the uniform implementation of InterCare’s relevant clinical guidelines and organizational policies and procedures in assigned areas.
  • Focuses self and team members on functional outcomes for patients that improve quality of life and reduce unnecessary or inappropriate utilization of the health care system.

Technical Skills

  • Proficient in Microsoft Office Suite
  • Ability to use clerical and numerical skills in preparing final drafts and documents from raw data
  • Ability to create letters, forms, documents, presentations
  • Ability to run reports, analyze and interpret data, assist in implementation and maintenance of HR systems

Communication Skills

  • Possesses a professional level of written and verbal communication skills
  • Ability to communicate complex concept in a clear effective manner
  • Possesses excellent cross cultural communication skills and the ability to communicate to staff members at all levels

Physical Demands

Job duties are performed in the typical office environment of the organization, which requires ordinary ambulatory skills sufficient to visit other locations. The environmental factors and/or physical requirements of this position include the following:

  • Ability to stand, walk, stoop, kneel, crouch, frequently sit, and lift up to 25 lbs.
  • Requires good hand-eye coordination, reach with arms & hands, and finger dexterity, including ability to grasp, and manipulate (using keyboard, office equipment).
  • Visual acuity to use keyboard, operate office equipment, and read printed material and regularly required to talk and hear.
