**Position: Concierge Physical Therapist**

**Location: Houston, TX**

**Employment Type: PRN Flexible Hours**

We are currently looking for a dedicated and motivated individual with an enthusiastic attitude to join our team as a **PRN Physical Therapist in Houston, TX**. If you enjoy caring for patients from diagnosis through full recovery while using voice-based documentation then this is the opportunity for you! This position is hired through our partner, Luna, as a contracted PT to the Houston Concierge group.

Responsibilities:

* Perform initial and ongoing physical therapy evaluations according to APTA standards of practice as well as state standards.

* Develop treatment plan for each patient based on initial and ongoing evaluations including short and long term goals.

* Implement treatment plan on a consistent schedule and adjust this schedule and plan according to patient progress.

* Document patient progress according to approved medical documentation and facility documentation procedures.

* Evaluate mobility, strength, and current level of function, while setting goals that are scaled to the patient's abilities and are realistically attainable.

We are seeking:

* Graduate from a CAPTE accredited Physical Therapy Program

* 3 or more years of outpatient orthopedic experience

* Current state of Texas license, CPR certification

* Independent transportation

* Outgoing and energetic personality
