Description

Summary:

This position will assist in administering the Outreach and Enrollment program within the organization. The goals of the Outreach and Enrollment program are:

  • decrease barriers for patients accessing the healthcare system by enrolling patients with third party or government-sponsored health insurance programs.
  • increase continuity of services by helping patients maintain coverage. 
  • increase patient engagement and service utilization by working proactively with patients to schedule appointments to proactively manage chronic diseases and focus on disease prevention.
  • increase patient awareness of additional services within the organization and encourage the use of integrated services.

This position will demonstrate and maintain expertise in: 

  • eligibility and enrollment rules and procedures. 
  • the range of qualified health plan options and insurance affordability programs.
  • the needs of underserved and vulnerable populations; and
  • privacy and security standards. 

This position will also coordinate activities relating to: 

  1. Conduct public education activities to raise awareness about coverage options available under Medicaid, CHIP and the Marketplace.
  2. Help individuals understand and access affordability options.
  3. Provide information and assistance in a fair, accurate, and impartial manner.
  4. Provide information and assistance in a manner that is culturally and linguistically appropriate to diverse communities and accessible to individuals with disabilities; and 
  5. Provide referrals to any applicable office of health insurance consumer assistance or ombudsman established under Section 2793 of the PHS Act to address consumer grievances, complaints, or questions about their health plan, coverage, or a determination.

Requirements

Minimum qualifications:

1. Must be 18 years of age or older.

2. Must be able to travel with the use of a personal vehicle.

3. Must possess the ability to read and interpret documents.

4. Must possess the ability to write routine reports and correspondence.

5. Must possess the ability to speak effectively before groups and actively engage the general public in outreach situations.

Knowledge, Skills, and Abilities Required:

General understanding of public relations and outreach, insurance coverage options for individuals and businesses, adept at dealing with diverse demographics, ability to manage other staff and volunteers. Ability to work flexible hours around community opportunities.

Duties and Responsibilities:

  1. Assist in the Development and implementation of the Outreach and Enrollment program. 
  2. Assist with defining program goals and the development of patient outreach program services including an annual work plan, education plan and appropriate policies and procedures. 
  3. Collaborate with other staff and volunteers.
  4. Maintain schedule and calendar of O&E community events.
  5. Assist with patient enrollment efforts and new patient outreach activities.
  6. Conduct relevant public education activities.
  7. Develop systems that ensure individuals understand and access affordability options.
  8. Represent Heritage Health at relevant public events.
  9. Enhance employee engagement by providing the resources, support and tools to promote efficiency, innovation and teamwork. 
  10. Serve as a representative of the organization to external agencies as directed.
  11. Engage in community development and relationship building with external partners.
  12. Public speaking. 
  13. Act as representative for Charity Reimagined. Including receiving and forwarding patient applications to Charity Reimagined, then following up with patients upon approval. 
  14. Complete and process Extra Help Applications for patients 
  15. Participating in Mary Ellen Scholarship Committee 
  16. Enrolling eligible patients in Everyday Wellness Program
  17. Regular and predictable attendance is an essential function of this position.
  18. Other duties as assigned.
