Improve the overall health of the communities we serve by providing primary medical services at the Heritage Health Nurse Practitioner Residency Clinic.

Requirements
Duties and Responsibilities:
1. Participates in all aspects of residency program including patient encounters, didactic and specialty rotations, evaluations and special projects.
2. Treats a mixed patient population, across gender, age and ethnicity, as well as insure and uninsured patients.
3. Examines patient to determine general physical condition. Performs physical examinations and preventive health measures within prescribed guidelines.
4. Orders or executes various tests, analyses, and diagnostic images to provide information on patient’s condition.
5. Analyzes reports and findings and diagnoses condition.
6. Prescribes or recommends drugs or other forms of treatment such as physical therapy, inhalation therapy, or related therapeutic procedures. Orders inoculations and vaccinations for patients to immunize from communicable diseases. Performs minor surgery. Refers patients to medical specialist or other practitioner for specialized treatment.
7. Advises patients concerning diet, hygiene, and methods for prevention of disease.
8. Records and/or signs off on physical findings. And formulates plan and prognosis, based on patient’s condition, in patient’s electronic chart in accordance with established standards.
9. Reports births, deaths, and outbreak of contagious diseases to governmental authorities.
10. Participates in provider staff meetings to develop problem-solving techniques and acquire team-building skills.
11. Participates in peer reviews.
12. Participates in clinic quality initiatives as requested.
13. Participates in program requirements including specialty rotations, didactic presentations, case presentations and project work.
14. Heritage Health staff have an active role in our Patient Centered Medical Home model of care. This role is designated as part of the Heritage Health PCMH Care Team.
15. Regular and predictable attendance is an essential function of this position.
16. Performs other job-related duties as assigned.
