Park Place Behavioral Healthcare - Master is Hiring a Targeted Case Manager, Adult/Child Near Kissimmee, FL

SUMMARY:

This is a professional-level position designed to serve consumers at risk of psychiatric institutionalization or hospitalization, consumers who are on discharge status in institutions or short-term facilities, consumers who have been admitted or discharged from state hospitals within the past two years, and consumers with multiple short-term inpatient admissions.

Essential Duties and Responsibilities:

  • Assessment of high risk consumers in terms of current and potential strengths, weaknesses and needs.
  • Develop holistic service plans with consumers addressing the broad needs characteristic of the service population.
  • Link and refer consumers to all internal and external services needed.
  • Ensure the proper documentation of progress notes related to requirements of Medicaid, licensing and accrediting agencies.
  • Coordinate cases to facilitate implementation of the service plan.
  • Manage case records to assure that proper documentation is contained and meets applicable standards.
  • Initiate revisions and updates to reflect additions or changes in Medicaid, licensing and accrediting agencies.
  • Monitor consumer’s progress with respect to the provisions of the service/treatment plan.
  • Evaluate impact of services on consumer’s quality of life.
  • Advocate on the behalf of persons served to ensure that their needs are met.
  • Provide supportive crisis intervention to consumer’s when needed.
  • Other duties assigned by the Director of Case Management.

QUALIFICATION REQUIREMENTS:

  • Bachelors, degree in a mental health or related field.
  • Individual must have ability to communicate effectively, both verbally and in writing with clients, in staffing meetings, and with supervisors.
  • Knowledge of the local community resources desirable.
  • Ability to work a flexible schedule when necessary.
  • Excellent organizational skills
  • Ability to prioritize the needs of the clients.
  • Able to work independently.

CORE COMPETENCIES:

  • Maturity and sensitivity to the special needs of PPBH clients while possessing the knowledge of behavioral health and medical conditions.
  • Possess the ability to document the services provided to customers in accordance with established governing standards in a timely, legible, accurate and complete manner.
  • Ability to communicate client needs and information.
  • Ability to utilize and navigate an electronic health record and Microsoft office software.
  • Must possess the ability to maintain client confidentiality

REQUIREMENTS:

  • A valid Class E Drivers’ License for positions that require driving personal or Agency vehicle on Agency business.
  • Must maintain a minimum Bodily Injury & Property Liability insurance limit of at least $50,000 for positions that require driving personal own vehicle (POV) on Agency business. (Proof Required)
  • PPBH is a 24/7 Agency. All employees must be available as needed when required.

COMPUTER SKILLS: Must have Intermediate to advanced experience in MS Office Suite (Outlook, Word, Excel, PowerPoint), the use of personal computers, and standard office productivity software.

LANGUAGE SKILLS: Ability to read and interpret documents such as safety rules, operation and maintenance instructions, and procedure manuals
