Job Description
HCP Talent is collaborating with a Long Term Care/Rehabilitation facility (120 beds) in Ithaca, NY that is seeking seeking a MDS Coordinator Registered Nurse (RN) for a full-time day shift M-F position.
MDS RN Duties and Responsibilities:
- Assess and appraise the caliber of care being administered to long-term residents.
- Initiate comprehensive interviews and evaluations of incoming residents.
- Help develop meticulously structured care plans encompassing both short-term and long-term objectives for advancement.
- Spearhead interactions with multidisciplinary treatment teams, caregivers, and families through productive meetings.
- Participate in Nursing management team meetings and seamlessly interact with all levels of Nursing staff.
- Conduct enlightening in-service education sessions on MDS, CAAs, and Triggers.
- Adhere diligently to the MDS/Care Planning schedule.

Qualifications & Requirements:
- Valid New York RN license
