Assesses patient needs for Speech Therapy and related therapeutic services in the home care setting, develops treatment plans and goals in consultation with other treatment team members, and conducts therapy programs designed to improve functioning and enhance patients well-being.
Schedule: per diem

• All start of care responsibilities are conducted by our Nursing staff

Educational / Minimum RequirementsMust have a minimum of a Bachelor’s Degree in Speech Therapy.
Must have at least 1 years of experience in Speech Therapy.
State/Federal Mandated Licensure Or Certification RequirementsCurrent Connecticut Licensure. CEU’s as mandated by the State of Connecticut.
Must have a valid CPR certification or be willing to obtain upon hire - American Heart Association or American Red Cross.
Must be willing to be fingerprinted for the CMS database system in the State of Connecticut.
Bristol Hospital Mandated Educational RequirementsGeneral orientation at time of hire. Fire/Safety/Infection Control annually. Other programs as mandated by Hospital. CPR annually.
Special RequirementsExcellent written and oral interpersonal communication skills.
For Home Care Agency: Must have a car to drive to home visits and a valid
Connecticut driver’s license in good standing; $100,000-$300,000 automobile liability insurance.
Physical RequirementsProvides direct patient care to patients.
Work EnvironmentOccasional exposure to body fluids, occasional exposure to hazardous chemicals. Exposure to stressful situations. Personal contact and varied levels of customers in the home setting.
