• RN Case Managers provide direct patient care and services
• Provide Admissions, Home Visits and follow up
• Coordinate care with therapies for multidisciplinary approach
• Delegate care to LPNs and HHAs on your patient care team
• Facilitate patient and family independence in managing their comorbidities
• Promote wellness and goals and interventions to attain goals of care
• Electronic documentation
• Experience in Home Health Care, including Matrixcare a plus, 1-year minimum Nursing Experience Required
