Qualifications of the Admissions Coordinator/Social Worker:

  • Associates or Bachelor's Degree in Social Work required
  • Prior admissions experience in health care facilities
  • Knowledgeable in Medicare requirements and insurance providers i.e. HMO rules, Medicare rules, RUGS II & III, and Medicare prescription plans.
  • Must have patience, tact, cheerful disposition, and enthusiasm, as well as must be willing to handle residents, staff, and visitors, based on whatever maturity level at which they are currently functioning.
  • Must successfully complete a post offer, pre-employment criminal background check and physical / drug screen.

Admissions Coordinator Purpose:

The primary purpose of the Admissions Coordinator position is to establish referral sources and to obtain required information and admit residents in an efficient manner, in accordance with established policies and procedures, and as directed by your supervisor.

As the Admissions Coordinator you must be delegated to the administrative authority, responsibility, and accountability necessary for carrying out your assigned duties.

Responsibilities of the Admissions Coordinator:

  • Timely conversion of referrals to admissions, and facilitation of an effective resident / patient admission process. They are also the community liaison for non-medically related public contact.
  • Obtain required information and admit residents in an efficient manner, in accordance with established policies and procedures
  • Completes weekly activity and progress reports (written and verbal) reflecting budget.
  • Manages the admissions process, to include the use of all required admissions paperwork, timely and accurate completion of paperwork, courteous treatment of residents and family members, and implementation of qualifying expectation setting guidelines where appropriate.
  • Provides pertinent information about incoming residents to all relevant departments within the facility.
  • On a weekly basis, manually enters referrals into the appropriate referral tracking software completing each section in an accurate and complete manner.
  • Exemplifies a working knowledge of the Levels of Care provided by the Facility, special programs and designated referral priorities for the Facility.
  • Generates appropriate referrals from targeted accounts identified in sales plans.
  • Conducts external sales activity with current referral sources to increase census and maximize conversions.
  • Analyzes referral log tracking trending information, denial statistics, and additional referral/admission data.
  • Conducts effective tours to promote conversion to admission.
  • Coordinates and conducts customer service training for Facility and staff.
  • Coordinates public relations, marketing and advertising activities for assigned market.
  • Additional job duties assigned as related to this position.
