Job Type: Part-Time

Compensation: At Guaranteed, we aim to offer equitable and competitive compensation that demonstrates the value we place in our employees and our appreciation for the work they perform; the base salary range for this role is likely to fall between $45-55 per hour depending on experience.

Summary

The Admissions Nurse is a registered nurse whose primary duty is to work closely and collaboratively with family members, members of the hospice interdisciplinary group, and other clinical staff to evaluate hospice eligibility and complete an initial assessment for patients who meet the criteria.

The Admissions Nurse will use their knowledge of LCD guidelines, pain and symptom management, and knowledge of end-of-life processes, the role and philosophy of hospice in order to educate these patients, family members, and caregivers about hospice services. This position will work closely with the hospice IDG team, Primary Care clinicians, and other clinical staff.

Job Responsibilities

  • Evaluate and determine if patients meet hospice criteria

  • Admit patients to hospice services

  • Completes admissions paperwork within a timely manner (assessments, orders, etc.)

  • Conducts initial meetings with patients and families to inform them about hospice services and obtains consent for care

  • Performs follow-up meetings and phone calls from information visits

  • Documents visits and phone calls with patients, families, and referral sources

Maintains confidentiality of all information related to patients, medical staff, employees, and as appropriate, other information. Demonstrates service excellence and positive interpersonal relations in dealing with others, including patients, family members, caregivers, employees, clinicians, volunteers, and community members, so that productivity and positive relations are maximized.

Relevant Experience

  • Active Registered Nurse licensed in California (BSN preferred but not required)

  • Excellent verbal, written, interpersonal, and organizational skills

  • 2 to 5 years of experience as a registered nurse in healthcare, 2 years hospice experience (required)

  • Proficiency in working with electronic medical record (EMR) system

Skills and Toolset You’ll Need to Bring

  • Ability to work effectively and collaboratively in an interdisciplinary team-oriented environment

  • Strong conflict resolution, multi-tasking skills, creative thinking, and problem-solving skills are critical to success in this role

  • Knowledge of Medicare, Medicaid, and insurance benefits a plus

  • Knowledge of Hospice LCD guidelines
