Mobile Healthcare Nurse (RN) - Full Time ($5,000 Sign-On Bonus!)

As the Mobile Healthcare Nurse , you will provide patient-centered care in close collaboration with the primary care provider, medical assistant, and multidisciplinary departments; including but not limited to preventive, sick, and supportive services to adult and pediatric patient in the Mobile Healthcare Program.

Salary Range: $31.00 - $36.00

Job Duties

  • Gathers patient and/or family to obtain a comprehensive medical and psycho/social history.
  • Effectively triage acute patient’s complaints. Provides accurate, detailed and effective medical assessments to provide safe, efficient and accurate nursing knowledge.
  • Supports the patient in improving access to health resources and social services.
  • Supports the primary care team in meeting performance measures and quality improvement goals.
  • Collaborates with all members of the Care Team to coordinate and manage care for the patient.
  • Provide a Culture of Safety by providing safe, high quality care to patients.
  • Performs health screening procedures under the direction of the Provider including lab procedures (e.g., lead screening, hematocrit, immunizations, etc).Participates in the preparation of patient’s charts. As directed by the Provider, assists with the patient intake and discharge procedures.
  • Proactively engage patients in their care with strategies to improve self-management, including assisting patients in identifying and improving upon self-management goals. Provide patient/family/caregiver education and act as patient advocate in the delivery and coordination of patient care.
  • Collaborate with all clinical staff to ensure care coordination elements of a Patient Centered Medical Home are being met.
  • Facilitate coordinated continuity of care for patients, thus reducing or eliminating fragmentation, duplication, and gaps in treatment plan, especially as a result of care transitions.
  • Demonstrates teamwork and collaboration with provider and other members of the care team.
  • Coordinates Screenings for community members.

Qualifications

  • Current New Hampshire Nursing License required
  • Experience in primary care and ambulatory health care setting preferred
  • Current BLS certification required

Mobile Healthcare Sites: Cross Roads House, Portsmouth, NH; St. John’s Episcopal Church, Portsmouth, NH; City Hall, Dover, NH; Community Action Partnership (CAP), Dover, NH; Salvation Army, Rochester, NH; St. Vincent DePaul Community Assistance Center, Exeter, NH.

Hours Per Week: 40 (8:00am – 5:00pm)

Days: Monday – Friday

Job Status: Full-Time (Non-Exempt)

This position is eligible for one-time sign-on bonus of up to $5,000.00.
