Qualifications:
- Bachelor's degree in social work or related field
- State licensure as a Social Worker (LCSW or LSW) Preferred but not required
- Minimum of 2 years of experience in a healthcare or long-term care setting
- Strong interpersonal and communication skills
- Knowledge of relevant laws, regulations, and policies pertaining to long-term care facilities
Duties:
- Assessment and Care Planning: Conduct comprehensive assessments of residents' psychosocial needs and develop individualized care plans in collaboration with interdisciplinary team members.
- Support Services: Provide emotional support and counseling to residents and their families to address social, emotional, and psychological concerns.
- Resource Referral: Connect residents and families with community resources, support groups, and other services to enhance their quality of life.
- Discharge Planning: Coordinate discharge planning and transition services to ensure a smooth transition for residents returning home or transferring to another care setting.
- Advocacy: Advocate for residents' rights and ensure that their voices are heard in decision-making processes related to their care and treatment.
- Documentation: Maintain accurate and up-to-date documentation of social service interventions, assessments, and care plans in compliance with regulatory standards.
- Team Collaboration: Collaborate closely with interdisciplinary team members, including nurses, therapists, and physicians, to address residents' holistic needs.
- Marketing: Market to the local community, hospitals, community groups, families, etc. to support admission rates and provide knowledge of Granger Nursing and Rehab’s services offered.
