- Coordinate and oversee the completion of the Minimum Data Set (MDS) assessments for all residents in a healthcare facility
- Ensure accurate and timely completion of MDS assessments in compliance with state and federal regulations
- Collaborate with interdisciplinary team members to gather necessary information for MDS assessments
- Review medical records, conduct resident interviews, and observe resident care to gather data for MDS assessments
- Utilize knowledge of Medicare guidelines and reimbursement systems to accurately code MDS assessments
- Monitor and track changes in residents' conditions to ensure appropriate updates to MDS assessments
- Collaborate with healthcare providers to develop individualized care plans based on MDS assessment data
- Provide education and training to staff members on MDS assessment process and documentation requirements

- Acute care experience, preferably in a long-term care or skilled nursing facility setting
- Strong knowledge of DRG (Diagnosis Related Group) coding and clinical documentation improvement processes
- Proficiency in ICD-10 coding system and medical coding practices
