Description
Case Manager
As our Case Manager, you will engage the client in creating and implementing an individual service plan, identifying their self-identified goals and objectives, service needs, strengths, barriers, support systems, and emotional triggers. Using this plan, you will link the client to community services and work with them directly to provide support and training on living skills such as grocery shopping, budgeting, home upkeep, meal planning, social skills, coping strategies, and self-monitoring their psychiatric symptoms. Through regular home visits, you will monitor the client’s progress and behaviors, and support the client’s goal attainment. This position includes client transportation as needed. For optimal client success, you will primarily meet with clients in the local community.
