Overview:

Wage Range: $40.78- $69.38

Per Diem Position

DEFINITION OF POSITION :

Assesses, delegates, coordinates, and evaluates the nursing care provided. Ensures provision of quality care for selected groups of patients through utilization of nursing process, established standards of care, policies and procedures. He/She initiates the Plan of Care and necessary revisions and is responsible for the overall care of the patient. He/she initiates appropriate measures to prevent disease exacerbation, promote safety, makes referrals to rehabilitative services and coordinates all services provided by other disciplines/organizations. The Home Health RN is directly responsible to the Director of Home Health.

Must be able to 1) perform a variety of duties often changing from one task to another of a different nature without loss of efficiency or composure; 2) accept responsibility for the direction, control, planning, of an activity; 3) make evaluations and decisions based on measurable or verifiable criteria; 4) work independently; 5) recognize the rights and responsibilities of patient confidentiality; 6) convey empathy and compassion to those experiencing pain, grief, and illness; 7) relate to others in a manner that creates a sense of teamwork and cooperation; 8) communicate effectively and therapeutically with people from every socioeconomic background; 9) incorporating guest relations in daily activities; 10) share responsibility for 24 hour call on a rotational basis with other Home Health staff RN's; 11) is well groomed and demonstrates appropriate and professional conduct, has access to a reliable motor vehicle during designated work hours.

Working Conditions:

Inside and outside environmental, not always protected from the weather temperature changes. Inside environmental changes are mainly those inside patient homes. Those can include temperature variations, smoke, unclean homes, various sizes and types of pits, and structural faults/barriers. Subject to frequent exposure to infection, contagious disease, and hazardous materials and equipment. Subject to combative patients, dysfunctional families and varying relationships. Variable noise levels. Also subject to rapid pace, multiple stimuli, unpredictable environment, and critical situations.

Physical Demands/Traits:

Must be able to 1) perceive the nature of sounds by the ear; 2) express or exchange ideas by means of the spoken word; 3) perceive characteristics of objects through the eyes; 4) extend arms and hands in any direction; 5) seize, hold, grasp, turn, or otherwise work with hands; 6) pick, pinch, or otherwise work with fingers; 7) perceive such attributes of objects or materials as size, shape, temperature, or texture; and 8) stoop, kneel, crouch, and crawl. Must be able to lift 50 pounds maximum with frequent lifting, carrying, pushing, and pulling of objects weighing up to 25 pounds. Continuous walking and standing. Must be able to identify, match, and distinguish colors. Rare lifting of greater than 100 pounds. Must be able to safely operate a motor vehicle.

Qualifications:

QUALIFICATIONS:

Education:

Required: Graduate of an accredited school of professional nursing. Evidence of continuing education appropriate for skill level and area of practice.

Desired: BSN

Experience:

Required: One year continuous RN experience in an acute care setting.

Desired: Previous Home Health experience and/or community health experience, or critical care.

Licenses, Certifications, and/or Registrations:

Current RN license in the State of Oregon; CPR certification; Maintains a valid Oregon's Driver's License and proof of current auto insurance and completion of a CPI training program.

Equipment/Tools/Work Aids:

Infusion pumps, PCA pumps, and other medical equipment, computer terminal printer, photocopier, and patient charts.

Specialized Knowledge and Skills:

Ability to provide care for the patient's age-specific, cultural, physical, and psychosocial well-being.
