Summary of the position: The Older Youth Services Community-Based Case Manager is responsible for providing home-based and community-based quality services for families involved with the Department of Child Services and/or Juvenile Probation through the Older Youth Services contract. The Case Manager will provide community-based and home-based services to clients in compliance with Service Standards and Protocol published by the Indiana Department of Child Services. It is the responsibility of each Older Youth Services Case Manager to know all of the published DCS standards for Older Youth Services that they are providing.

Requirements:

• Bachelor's degree in social work, psychology, sociology, or a directly related human service field from an accredited college.
