ollaborates with a multidisciplinary team, performing assessments, diagnosing patients, and medication management.EducationGraduate of an accredited school of nursing
Certifications / ExperienceCurrent, unrestricted license to practice as Registered Nurse Practitioner in the state the facility is located (maintain a current copy of license on file at the facility)
1 year of direct work experience, preferred
Active CPR certification
Appropriate certification to write prescriptions under the authority of the Collaborative Physician in accordance with state and federal guidelines.

Ability to obtain a security clearance, to include drug screen and criminal background check
Responsibilities include but not limited to:Conduct on-site or telehealth (site specific) chronic care clinics and sick call as needed
Ensure progress notes are documented in appropriate format are problem oriented, correspond to the therapeutic order and is dated, timed and legible
Adhere to the established formulary before using non-formulary
Regularly review cases that require ongoing medical/pharmaceutical attention
Maintain compliance with company policies and procedures and NCCHC standards where applicable
Assist in designating the level of care to be performed and the necessary equipment needed
Consult with multidisciplinary team to ensure patients receive adequate care
