Full-time Description WOUND CARE COORDINATOR (RN) - Full Time - Generations Oakton Pavillion, Des Plaines, IL Generations at Oakton Pavillion in Des Plaines, IL, has an Immediate Opening for Full Time Wound Care Coordinator, RN. Salary of up to $95k, Depending on Qualifications, Plus Full Benefits, including 401k w/Company Match, HSA, Teledoc, Choice of Insurance Plans, and much more! The Schedule will be Monday-Friday, However, some Weekend Flexibility will be Required for Training and Coverage. *Wound Care Certification Preferred, but Not Required. Plan, implement and set priorities for wound care and prevention according to residents needs
Medication application to existing wounds
Educate patients and their families to care for their own wounds
Care for patient's surgical incisions, any pressure sores or any wounds caused from disease or injury
Skin evaluation for pressure sore prevention
Stocking the treatment carts
Complete MDS, careplan
Work with wound physician on weekly rounds
IND1 Requirements of the Wound Care Coordinator (RN):
Current RN license from the State of Illinois
Nursing experience at a SNF
Wound Care experience required
MDS and Care Plan experience a big plus
Wound Care certification (WCC) or VOHRA preferred
Personable and professional with residents, families, and staff
Maintain confidentiality of all residents' information in compliance with HIPAA standards
Must have compassion, understanding and tolerance for older adults
