Full Time | Day Shift | 8am - 5pm | Monday - Friday

** Pay rate between $65,000 - $90,000/year; based on experience! **

Requirements for a Glenmere RN Case Manager:

  • Graduation from an approved RN school of nursing, current New York State Registration and License.
  • At least three years' experience working with an adult population providing supportive care services in an adult care facility or long-term care building.
  • Two years of direct supervisory experience is preferred.

Responsibilities for a Glenmere RN Case Manager:

  • Ensures that the Memory Care SNALR neighborhood provides residents with quality of care and services, ensuring the highest level of functioning for each resident.
  • Dementia specific focus with collaboration in programing to meet the ever-changing needs of the resident.
  • Ensures that the Assisted Living ALR and Enhanced Care EALR provides an environment that encourages independence and aging in place as appropriate.
  • Maintains documentation in accordance with the Department of Health Guidelines for all levels of Assisted Living.
  • Formal and informal evaluations.
  • Ongoing assessment of needs and implementation of services to meet those needs.
  • Responsible for overall development, implementation, and review of Individual Service Plan (ISP) and review biannually with all residents.
  • Focus on the total well-being of the resident, which includes social, family, financial, medical, mental health, key life events, dietary, and spiritual.
  • Ensure quality of care through regular daily reviews of required documentation, physician's orders, chart audits and overall individual service plan completion and review.
  • Conduct pre-admission/re-admission evaluations (DOH-4397) based on resident needs.
  • Review necessary pre-admission & admission paperwork (DOH-3122, DOH-4397, Consumer Information Guide, Resident Handbook, etc.) with the resident and/or responsible party.
  • Ensures that newly admitted residents are appropriately orientated to the ALR, EALR, and SNALR.
  • Complete the Admission Agreement with the resident and/or responsible party.
  • Partners with the Nurse Manager to maintain Department of Health regulated records, census, incident report, admission, and discharge log.
  • Obtain POA, HCP and contact information as necessary.
  • Encourage residents to maintain independence and participate in activities.
  • Maintain records documenting the case management needs of each resident and case management activities performed.
  • Coordinate care with other providers.
  • Provide family with support as needed and keep updated as appropriate on resident's status.
  • Coordinate family/resident meetings.
  • Assist residents and families in planning and arranging alternative care setting, as desired/needed.
  • Communicate with families, physicians, and other care providers in emergency discharge situations.
  • Assist Nurse Manager with staff scheduling completion and payroll documentation.
  • May also be responsible for providing on-floor staff coverage on weekends, evenings and nights as required and medication pass when staffing levels are low.
  • Assist Nurse Manager with implementation and completion of monthly in-services and additional training.
  • Attend and participate in Resident & Family Council Meetings.
  • May assist with responsibilities for management of the Resident Care Department in the absence of the Nurse Manager.
  • Responsible to participate in on-call rotation.
  • Other duties as deemed necessary and appropriate or as may be directed by your supervisor.
  • Responsible for activities of daily living being performed in accordance with care plan, procedures, and mission and goals.
  • Monitors daily care and associated documentation to ensure standards are met and maintained.
  • Participates in recruitment, selection, and training of household staff with input and assistance from other leaders including Assistant Executive Director, Nurse Manager and Dining Services Manager.
  • Responsible for delivering and/or coordinating staff training specific to DCS (both new employee training and ongoing) to assure competence in care techniques and documentation and adherence to all procedures and policies.
  • Responsible for completion and coordination of all resident assessments with responsible parties, to ensure adherence with all timelines and procedures and to assure appropriate placement in the ALR according to admission and retention criteria, with support from the other managers.
  • Responsible for assuring ISP Plan reflects the resident's background, cognitive level, personal preferences and routines, other assessment information and reflects care program goals.
  • Responsible for assuring the ISP is executed appropriately by staff and coaching/assisting staff as required (and in coordination with nursing leadership as required).
  • Acts as main liaison with family/responsible party developing and maintaining excellent family relationships, conducting frequent family updates, and attending care plan conferences.
  • Assist with facilitating a safe and successful transfer or admission of a resident to their environment, in coordination with other personnel.
  • Responsible for ensuring the environment is well-maintained, safe, and homelike in accordance with appropriate department managers.
  • Participates in care facility events and community events.
  • Responsible for assurance of maintenance of resident dignity and respect.
