CASE COORDINATOR

Job Category: Behavioral Interventionist Program

Job Status: Full-time

Reports to: Behavioral Interventionist Program Manager

Job Summary: The Case Coordinator provides supervision to frontline Behavioral Interventionist (BI) staff and case management to families served in the BI Program. 

Responsibilities

Case Management 

  • Ensure family satisfaction, providing proper support, advocacy, resources, & communication
  • Complete assessments, scoring, tracking progress; adapting services to the needs of the client
  • Attend court & other meetings
  • Help match BIs to homes, resolving issues, participate in team meetings
  • Review Incident Reports with Program Manager; follow-up with family
  • Complete/facilitate Referral Triage, Intake, Meet & Greets for each family
  • Complete/submit Recommendation/Approval Letters for services
  • Create Individualized Behavioral Plan for each client; review & update as needed
  • Maintain communication with all parties when placed on waitlist; during intake, approval, and BI hiring process
  • Complete meetings with each family, with the BI
  • Submit renewals on time
  • Maintain client database profiles
  • Submit End of Month Reports to Supervisor
  • Complete Case Closure & Aftercare Follow-up meetings & documentation
  • Review BI Quarterly Summaries & provide feedback to staff

Administrative Leadership & Supervision 

  • Responsible for BI staff satisfaction, build a strong sense of team
  • Ensure proper support/advocacy for frontline staff
  • Participate in the BI hiring process
  • Training for new BIs and All-Staff Direct Care Services Employee Meetings
  • Conduct client/staff matchings with Program Manager
  • Conduct on-going evaluation conferences with BIs
  • Inform Program Manager of concerns (work performance/client issues)
  • Provide in-home supervision for BI staff
  • Track all missed Shift Notes
  • Complete corrective actions & terminations with Program Manager
  • Update/track employee timecards
  • Provide support to BIs during OHI Investigations
  • Work with a diverse team to effectively/efficiently serve clients
  • Build/maintain relationships with partner agencies & community stakeholders
  • Strong, open communication with the team re: services delivered, concerns/questions
  • Maintain CPR/First Aid certification
  • Collaborate with branch office(s) for program fidelity
  • Attend professional development trainings

Outcome Planning/PQI 

  • Complete agency reports & statistics
  • Ensure procedures are in place to track/report program outcomes & quality assurance
  • Ensure all resource provider files are in compliance
  • Complete Critical Incident Reports with Program Manager

Qualifications & Preferred Skills 

  • MA/MS (or BA/BS 2 years in non-profit social service) in social work, psychology, public administration, or related human services field
  • Previous experience with at-risk populations
  • Familiarity with child welfare system
  • Setting priorities, creating efficiencies, guiding investment in people & systems
  • Understanding of program development, implementation & evaluation
  • Organizational & planning skills
  • Commitment to principles of support/advocacy for foster/adoptive families
  • Strong verbal & written communication skills
  • Leadership, team management skills
  • Collaborate/advocate for positive outcome
