The CNA Performs essential personal care duties for both Home Health and Hospice patients in their homes. Attempts in every possible way, in working with individuals and families, to assist them to assume responsibilities toward self-care. Helps the well family members to carry their share of the responsibilities and encourages the disabled member to participate in the life of the family, in so far as able.

Responsibilities:
  • Performs duties as assigned, including baths (sponge, tub, shower), hair care (bed, sink, tub) oral hygiene, nail care, foot care, skin care, toileting and elimination, pressure relief, dressing patient, feeding and transferring patient.
  • Takes accurate vital signs, including reading and recording temperature, pulse and respirations.
  • Describes the activities of daily living and demonstrates the ability to use techniques to assist the patient to meet these activities.
  • Notifies appropriate licensed staff when a patient complains of pain or develops change in condition.
  • Demonstrate range of motion techniques and ability to position patients in bed with proper body alignment.
  • Assists patients with transfers and ambulation as necessary.
  • Performs all aspects of patient care in an environment that optimizes patient safety and reduces the likelihood of medical/health care errors.
  • Supports and maintains a culture of safety and quality including basic infection control procedures.
  • Understands Hospice philosophy of death and dying and pain control.
  • Understands and verbalizes adequate nutrition and fluid intake parameters.
  • Demonstrate knowledge of physical, emotional, and developmental needs of and ways to work with populations served by hospice including respect for patient, privacy and property.
  • Recognizes and implements emergency procedures as needed.
  • Identifies and communicates findings to the supervising RN, Case Manager or Clinical Supervisor.
  • Demonstrates the ability to chart according to professional standards.
  • Works as part of the interdisciplinary team.
  • Completes patient documentation on time.
  • Attends in-service and staff meetings as needed.
  • Completes assigned training on time.
  • Completes annual education requirements.
  • Maintains patient confidentiality at all times.
  • Performs other duties as assigned.
  • Possess the ability to follow written and oral instructions.
  • Demonstrate knowledge and skills necessary to provide care to the geriatric and adult population.
  • Compassion and empathy for patients.
  • Attention to detail regarding a patients problems to analyze what they need to ease their pain and to prevent further injury.
  • Interpersonal skills to build positive relationships with patients, families and staff.
  • Communication skills to interact clearly with doctors, family members, patients and the entire team of care.
Qualifications:
  • Graduate of an accredited High School or equivalent preferred.
  • Home Health/Hospice Aide certificate obtained by: Completion of a certified home health aide course or Certified by the State Department of Health Services.
  • Successful completion of a State-established or other training program that meets CMS requirements (484.36(a)) and a competency evaluation program or State licensure program that meets CMS requirements (484.36(b) or (e)). Note: An individual is not considered to have completed a training and competency evaluation program or a competency evaluation program if since the individual's most recent completion of the program(s) there has been a continuous period of 24 consecutive months during none of which the individual furnished HHA services for compensation.
  • On-the-job instruction relative to specific patient needs and continuing inservice training, so that patient safety may be assured and the personal care and homemaking needs of the patient are adequately met.
  • Completion of a nurse aide training and competency evaluation program approved by the State as meeting the requirements of 483.151 through 483.154, and is currently listed in good standing on the State nurse aide registry. Note: An individual is not considered to have completed a training and competency evaluation program or a competency evaluation program if since the individual's most recent completion of the program(s) there has been a continuous period of 24 consecutive months during none of which the individual furnished Hospice services for compensation. If there has been a 24-month lapse in furnishing services, the individual must complete another program.
  • Valid state drivers license and reliable automobile, current automobile insurance and being willing to operate personal vehicles necessitated by nature of job.
  • Current TB testing.
  • Current CPR card.
  • Listed on the nurse aide registry with no findings relating to client abuse, neglect, or misappropriation of client property.
