Description
JOB SUMMARY
Under the supervision of the Interim Housing Site Manager, the Interim Housing Case Manager is responsible for carrying a caseload of up to 25 participants at the Sanctuary Shelter. The Interim Housing Case Manager performs assessments and develops Individual Service Plans for participants. The Interim Housing Case Manager will meet with participants at minimum once a week to implement or review the participant's Individual Service Plan. The Interim Housing Case Manager collaborates with Program Leadership to help the organization maintain compliance with County, State and Federal regulations. The Interim Housing Case Manager understands ECS' strategic vision and acts in accordance with the organization's mission.
PRIMARY DUTIES AND RESPONSIBILITIES:
  1. Manage a caseload of up to 25 participants.
  2. Perform Individual Service Need assessments.
  3. Provide back-up coverage for other case managers, as directed by the Site Manager.
  4. Collaborate with the participant to develop an Individual Service Plan that identifies needs with the primary focus being housing.
  5. Create an action plan to assist the participant with accomplishing goal(s).
  6. Report participant progress to Program Leadership as required.
  7. Provide case management linkage services and support for housing, income, primary care, substance use treatment, behavioral health, legal, employment, clothing, and other resources.
  8. Meet with participant at minimum once a week and document sessions.
  9. Track and encourage participants to attend scheduled appointments.
  10. Maintain accurate records by entering documentation of all assessment, services, crisis management, and discharge services into various electronic systems for all participants in compliance with ECS practices.
  11. Maintain up-to-date documentation in participant files regarding services provided in a complete, confidential, and professional manner.
  12. Assist participants with planned exits from the program to engage in comprehensive discharge planning, that includes referrals for case management, housing, food, clothing, medical treatment, detox, and/or other services as necessary and appropriate.
  13. Adhere to agency, state, and funder regulations.
  14. All other duties.

Qualifications
REQUIRED QUALIFICATIONS :
  1. Associate degree in psychology, counseling, social work, or another related field and 3 years of relevant work experience
  2. CCSF Community Health Worker Certificate and 2 4 years of relevant work experience
  3. High school diploma/GED and 4 years of relevant work experience
  4. Experience working in integrated service teams using person-centered harm-reduction and trauma-informed approaches.
  5. Strong background working with individuals experiencing homelessness.
  6. Effective communication skills (written and oral).
  7. Excellent organizational and time management skills.
  8. Strong administrative and organizational skills. Must possess excellent charting, recordkeeping, and professional writing skills.
  9. Knowledge of Microsoft Office programs and experience documenting services into electronic record systems.
  10. Must be able to problem solve and quickly assess/respond to a variety of situations, including medical emergencies, psychiatric emergencies, conflict resolution and de-escalation interventions, as well as other types of crises.
  11. Operate with cultural humility in a diverse program and staff population.
  12. Excellent working knowledge of homelessness resources in San Francisco.
  13. Possess a strong knowledge of and sensitivity to adults with substance abuse, domestic violence, incarceration, HIV/AIDS, mental health including co-occurring disorders, and history of trauma.
  14. Must have the ability to welcome and respect co-workers and participant populations as unique individuals with different backgrounds, perspectives as well as differences in personalities, skill sets, life experiences, and knowledge bases.
  15. Lived experienced preferred, but not necessary.
As a Bilingual (Spanish-speaking) Case Manager, must be fluent in both written and oral communication and comprehension in Spanish.
OTHER:
Must secure finger image screening, show proof of COVID vaccination including booster, and annual TB screening. This position requires long periods of sitting at a desk and/or computer station. Will honor and abide by Episcopal Community Services rules and regulations, including confidentiality
