ESSENTIAL JOB FUNCTIONS:

A. Role Responsibilities Administrative & Personnel:

1. Uses the wristband or photo card file to identify residents before performing procedures, serving meals, etc., as necessary

2. Records all entries on flow sheets, notes, charts, etc., in an informative and descriptive manner

3. Uses only authorized abbreviations established by this facility when recording information

4. Reports all changes in residents condition to the Nurse Supervisor/Charge Nurse as soon as practical

5. Performs all assigned tasks in accordance with our established policies and procedures, and as instructed by supervisors

6. Follows work assignments, and/or work schedules in completing and performing your assigned tasks

7. Notifies the facility when you will be late or absent from work in accordance with policies

8. Meets with your shifts nursing personnel on a regularly scheduled basis to assist in identifying and correcting problem areas, and/or the improvement of services

B. Role Responsibilities Admission Transfer, and Discharge:

1. Ensures that the residents room is ready for receiving the resident (i.e., bed made, name tags up, admission kit available, etc.)

2. Introduces resident to his/her roommate, if any, and other residents and personnel as appropriate

3. Makes resident comfortable (i.e., put to bed, get water, etc.)

4. Inventories and marks residents personal possessions as instructed

5. Stores residents clothing

6. Assists residents with packing their personal possessions when they are being transferred to a new room, or when being discharged

7. Transports residents to new rooms or to the receiving area

C. Role Responsibilities Personal Nursing Care:

1. Participates in and receives the nursing report upon reporting for duty

2. Assists residents with daily dental and mouth care (i.e., brushing teeth/dentures, oral hygiene, special mouth care, etc.)

3. Assists residents with bath functions (i.e., bedbath, tub or shower bath, etc.) as directed

4. Assists residents with dressing/undressing as necessary

5. Assists residents with hair care functions (i.e., combing, brushing, shampooing, etc.)

6. Assists residents with nail care (i.e. , clipping, trimming, and cleaning the finger/toenails, not including diabetic residents)

7. Shaves residents as needed

8. Keeps residents dry (i.e., change gowns, clothing, linen, etc., when it becomes wet/soiled

9. Makes beds (occupied and unoccupied)

10. Position bedfast residents in correct and comfortable position

11. Assists residents with bowel and bladder functions (ie., take to bathroom, offer bedpan/urinal, portable commode, etc.)

12. Maintains intake and output records as instructed

13. Keeps incontinent residents clean and dry

14. Checks and reports bowel movements and character of stools as instructed

15. Collects specimens as instructed (i.e., urine, sputum, stools, etc.)

16. Assists residents in preparing for medical tests (i.e., lab work, x-ray, therapy, dental, etc.)

17. Assists residents in preparing for activity and social programs (i.e., church services, parties, visitors, etc.

18. Assists in transporting residents to/from appointments, activity and social programs, etc., as necessary

19. Assists with lifting, turning, moving, positioning, and transporting residents into and out of beds, chairs, bathtubs, wheelchairs, lifts, etc.

20. Assists residents to walk with or without self-help devices as instructed

21. Performs restorative and rehabilitative procedures as instructed

22. Provides eye and ear care (i.e., warm/cold compresses, eye/ear instillation, cleaning eyeglasses/hearing aids, etc.) as instructed

23. Assists in preparing the resident for a physical examination

24. Weighs and measures residents as instructed

25. Measures and records temperatures, pulse, and respirations (TPRs), as instructed

26. Answers resident calls promptly

27. Ensures that residents who are unable to call for help are checked frequently

28. Checks each resident routinely to ensure that his/her personal care needs are being met in accordance with his/her wishes

29. Assists with the care of the dying resident 30. Provides post-mortem care as instructed

D. Role Responsibilities Special Nursing Care:

1. Observes and reports the presence of pressure areas and skin breakdowns to prevent decubitus ulcers (bedsores)

2. Reports injuries of an unknown source, including skin tears

3. Provides daily indwelling catheter care

4. Provides daily perineal care

5. Assists with the application of slings, elastic bandages, binders, etc.

6. Gives tepid sponge baths

7. Provides daily range of motion exercises and records data as instructed

8. Turns bedfast residents at least every two (2) hours

9. Performs special treatments as instructed

10. Observes disoriented and comatose residents; records and reports data as instructed

11. Turns in all medications found in the residents room/possession over to the Nurse Supervisor/Charge Nurse 1

2. Provides residents with reality orientation as instructed

13. Watches for and reports any change in room temperature, ventilation, lighting, etc.

E. Role Responsibilities Food Service:

1. Prepares residents for meals (i.e., takes to bathroom, washes hands, combs hair, raise beds, positions tables, places bibs, takes to/from dining room, etc.)

2. Serves food trays and assists with feed as indicated (i.e., cutting foods, feeding, assists in dining room supervision, etc.)

3. Assists residents with identifying food arrangements (i.e., informing resident with sight problem of foods that are on his/her tray, where it is located, if it is hot/cold, etc.)

4. Records the residents food/fluid intake and reports changes in the residents eating habits

5. Keeps residents water pitchers clean and filled with fresh water (on each shift), and within easy reach of the resident

6. Serves between meal and bedtime snacks

7. Performs after meal care (i.e., remove trays, clean residents hands, face, clothing, take to bathroom, brush teeth, clean denture, etc.)

8. Checks rooms for food articles (i.e., food in proper container, unauthorized food items, etc.)

F. Role Responsibilities Staff Development:

1. Attends and participates in scheduled training and educational classes to maintain current certification as a Nursing Assistant

2. Attends and participates in scheduled orientation programs and activities

3. Attends and participates in annual OSHA and CDC in-service training programs for hazard communication, TB management, and bloodborne pathogens standard

4. Attends and participates in advance directive in-service training programs as scheduled

5. Attends and participates in outside training programs

G. Role Responsibilities - Interpersonal Skills:

1. Must function independently and have flexibility, personal integrity, and the ability to work effectively with the residents, personnel, and support agencies

2. Is involved with residents, personnel, visitors, government agencies/personnel, etc., under all conditions and circumstances

3. Cooperates and communicates with inter-departmental personnel, as well as other facility personnel to ensure that nursing services can be adequately maintained to meet the needs of the residents

4. Creates and maintains an atmosphere of warmth, personal interest, and positive emphasis, as well as a clam environment throughout the unit and shift

H. Role Responsibilities - Safety:

1. Restrains residents in chair/bed only as ordered

2. Checks restrained residents at least every thirty (30) minutes

3. Releases restraints at least every two (2) hours for range of motion exercises, taking to bathroom, etc.

4. Keeps the nurses call system within easy reach of the resident

5. Notifies the Nurse Supervisor/Charge Nurse of any resident leaving/missing from the facility

6. Follows established safety precautions in the performance of all duties

7. Keeps residents personal possessions off the floor and properly stored

8. Keeps floors dry and reports spills immediately

9. Keeps excess supplies and equipment off the floor and stores in designated areas

10. Stores all tools, equipment, and supplies before leaving work area for breaks or at the end of the work day

11. Reports all accidents/incidents, safety violations, hazardous conditions, or defective equipment to the charge nurse immediately

12. Follows established smoking regulations and reports all violations

13. Follows established fire, disaster, and evacuation policies and procedures

14. Uses only supplies/equipment trained to use

15. Wears and/or uses safety equipment and supplies when lifting or moving residents (back brace, mechanical lifts, etc.)

16. Operates all equipment and use all supplies in a safe manner and according to established procedures

17. Notifies supervisor of equipment and supply needs

18. Reports missing/illegible labels and MSDSs to supervisor

I. Role Responsibilities Infection Control & Sanitation:

1. Washes hands before and after performing any service for the resident

2. Participates in appropriate in-service training programs prior to performing tasks that involve potential exposure to blood/body fluids

3. Washes wheelchairs, walkers, etc., as instructed

4. Cleans, disinfects, and returns all resident care equipment to its designated storage area after each use

5. Reports any communicable or infectious disease to the DON and/or Infection Control Coordinator

6. Follows established isolation precautions and procedures

7. Follows established procedures in the use and disposal of personal protective equipment

8. Follows established policies concerning exposure to blood/body fluids

9. Washes hands before entering and after leaving an isolation room/area

10. Demonstrates proper hand-washing techniques and disposes of waste properly

11. Ensures that established infection control and standard precaution practices are maintained when performing nursing procedures

J. Role Responsibilities Care Plan:

1. Reviews care plans daily to determine if changes in the residents daily care routine have been made on the care plan

2. Informs the Nurse Supervisor/Charge Nurse of any changes in the residents condition so that appropriate information can be entered on the residents care plan

3. Ensures that your notes reflect that the care plan is being followed

K. Role Responsibilities Resident Care/Dignity:

1. Maintains the confidentiality of all resident care information

2. Ensures that you treat all residents fairly, with kindness, dignity, and respect

3. Ensures that all nursing care is provided in privacy

4. Ensures understanding of and compliance with all rules regarding residents rights

5. Knocks on residents rooms before entering

6. Reports all grievances and complaints made by residents to the Nurse Supervisor/Charge Nurse.

7. Reports all allegations of resident abuse and/or misappropriation of resident property

8. Honors the residents refusal of treatment request. Report such requests to your supervisor

EDUCATION/EXPERIENCE:

1. Must possess, as a minimum, a GED and Certified Nursing Assistant Course (120 hours)

2. Must be a licensed Certified Nursing Assistant in accordance with the laws of this state

3. Must be able to understand and carry out oral and written instructions

4. Must be able to read, write, speak, understand, and communicate effectively in English

5. Must be able to handle stressful situations and be flexible and adaptable

6. Must have patience, tact, a cheerful disposition and enthusiasm, as well as the willingness to handle difficult residents

7. Must be willing to seek out new methods and principles and be willing to incorporate them into existing nursing practices

8. Must be able to relate information concerning a resident's condition

9. Must possess the ability to make independent decisions when circumstances warrant such action

10. Must possess the ability to deal tactfully with personnel, residents, family members, visitors, government agencies/personnel, and the general public
