Beautiful Newly Remodeled Skilled Nursing Facility in Seattle seeks Resident Care Manager

Only On Call One Weekend per month

The Resident Care Manager (RCM) provides direct nursing care to residents and supervises the day-to-day activities performed by nursing assistants. Must evaluate and assess residents' needs, including physical and mental health, family and community resources, physical environment and finances. A plan of care for residents will be developed by the RCM based on a close assessment of resident’s requirements. Arrangements need to be made to implement that plan. Once these arrangements are in place, the RCM can coordinate all services to insure the resident's health, safety and general wellbeing. Often, there needs to be a continuous monitoring and periodic reevaluation to make changes as needed

Duties:

  • Participate in the development of a written plan of care for each resident and review resident’s medical and nursing treatments to ensure they are in accordance with the resident’s care plan and wishes.
  • Complete the planning, scheduling and revising of the MDS, including the implementation of RAPs and Triggers.
  • Make daily rounds to ensure that all nursing personnel are performing their work assignments. Review nurses’ notes to ensure that they are informative and descriptive of the nursing care being provided and the resident’s response to care and wishes.
  • Develop and maintain a good working rapport with all facility personnel to ensure that the needs of the resident are met. Participate in reviews of the discharge plans and prepare reports for the Care Plan Committee as directed.
  • Delegate, train, evaluate and support RN, LPN and CNA personnel. Provide direct nursing care as necessary.
  • Assist the Director of Nursing and fill in as needed.
  • Participate in facility surveys by authorized government agencies
  • Two years in supervisory/management position, including experience with budgets, staff development, training and scheduling
  • Working knowledge of MDS process, state and federal survey, and CMS is preferred.
  • Experience with Electronic Medical Records and computer documentation systems.

Qualifications:

  • Must possess a current RN or LPN license. License must be in good standings.
  • Prefer minimum 1 year of experience as a supervisor in a healthcare setting.
  • Must have training in rehabilitative and restorative nursing practices.

Salary $100,000.00 to $120,00.00 annually

Sign on Bonus paid $2000.oo at 30, 90, 150, and 180 days.
