Outpatient RN Navigator / Registered Nurse, Outpatient Cardiology

Shift: Full Time, 8am-5pm Monday-Friday

Location: The Emory Clinic, 5673 Peachtree Dunwoody Rd NE, Atlanta, GA

We are seeking a capable RN Navigator who has acquired broad experience in caring for patients, and has developed a sound understanding about the care of a particular patient population.

• Responsible for enhancing the patient experience by providing a seamless navigation process from diagnosis to survivorship.
• Responsible for collaborating with physicians and members of the interdisciplinary teams to triage, coordinate, and consistently manage patient care by educating, providing a link to research, serving as the primary point of contact for patients and families and providing indirect patient care.
• Works with patients and families to answer questions, provide emotional support and solve issues concerning the cycle of care.

MINIMUM QUALIFICATIONS:

• Graduate of an accredited school of nursing.
• BSN required.
• Two years nursing experience required, 1 year case management preferred.
• BLS required.
