Responsibilities

Purpose of Your Position:

The Patient Care Coordinator represents the Children’s Center with professionalism and dignity while exemplifying the Mission and Core Values. Collaborates with all members of the Leadership team to ensure effective communication for the safety and well-being of the residents. Responsible for the coordination and management of resident care activities, unit operations, and nursing personnel for the facility on the off shift hours.

 Position Functions and Responsibilities

• Creates and maintains an atmosphere of positivity, safety, warmth and support which encourages residents/families/staff to achieve their full potential.
• Provides staff with the support and guidance necessary to implement nursing practice and provide safe, appropriate goal-directed care.
• Initiates investigations of care issues and family concerns in a timely and thorough manner. This includes, but is not limited to, incident reports, medication errors, and abuse and neglect reporting.
• Interprets policies and procedures for nursing staff and other services. Provides education and training as needed related to policy and procedure.
• Provides input to Nurse Managers on a frequent basis regarding employee performance on their shift, and intervenes as necessary to provide coaching, counseling and/or progressive discipline.
• Ensures all neighborhoods are adequately staffed according to acuity and staffing guidelines.
• Frequent rounds on Neighborhoods to obtain clinical information on residents and to assist with any staff/family concerns.
• Provides accurate, objective, and clinical administrative reports.
• Follow-up on status of residents transferred to other facilities and conveys any updates to the oncoming shift.
• Other related duties as may become necessary or as directed by the Director of Nursing and/or department head.

Qualifications

• Bachelors of Science in Nursing; Master’s Degree preferred
• 5 years of nursing experience, 2 years pediatric experience and 3 years of Management/Supervisory experience
• New York State Nursing License
• BLS (required), PALS (preferred)

• Must have strong communication skills and the ability to lead and motivate the team.
• Comply with all state requirements and facility policies, procedures and standards of conduct.
• Have the ability to assess, plan, implement and evaluate nursing care according to individualized needs.
• Guides the staff and ensures tasks are being completed.

Salary Range:

$115,000/Yr to $130,000/Yr
