POSITION: Case Manager
PROGRAM: HIV Prevention
RESPONSIBILITIES: The Case Manager reports to Program Manager and assists the professional staff (e.g. Medical Care Manager (MCM)/Registered Nurse and Social Worker to implement the Medical Care Coordination (MCC) Program and the delivery of Medical and Non-Medical case management activities to patients. The Case Manager will be responsible for identifying, assisting, and connecting eligible patients at the Esperanza Clinic to the MCC program and other medical services/resources. The Case Manager will address clients’ socioeconomic needs and assist with client monitoring, linkage/referrals to services, updating care plan results, following up with clients, and tracking outcomes. Acts as the liaison between HIV Counseling and Testing sites and the medical clinic to ensure that new clients are enrolled in medical care seamlessly and in a timely fashion. To assist with patient management, the Case Manager will use a population health registry to enter patient data and to track patient’s outcomes.
The Case Manager will act as a liaison with patient to provider and/or other specialty clinics and referrals. The Case Manager will update patient care plans and results according to any testing and labs completed and enroll patients into Medical Care system. Will perform other tasks as assigned.
REQUIREMENTS: Must have six (6) months of experience working in a clinical settling; have knowledge/ experience in HIV/AIDS-related issues: able to research, identify and access community referrals. Experience working with people living with HIV.
OTHER REQUIREMENTS: Must possess a dependable automobile with insurance and a valid California Driver’s License. Must submit to and pass a drug test and a criminal background check as a pre-condition for employment. Must be able to provide proof of vaccinations for COVID-19 with boosters, MMR, TDAP, Hepatitis B and TB test.
QUALIFICATIONS: Bachelor’s degree in nursing, social work, counseling, psychology, sociology, or human services preferred; OR a combination of education and demonstrated work experience in the HIV field. Knowledge of HIV/AIDS, substance use, homelessness and mental illness, and other issues affecting client population; strong quality management skills, ensure compliance with all guidelines and regulations; fluent in English required, bilingual in
English/Spanish preferred.
