HYGIENE COMPENSATION

Compensation Plans (Current team members could choose one or remain where they are at)

·Flat Rate

• $35.00 hourly rate or $280 per diem rate

Hourly rate with Bonus

• $32.00 hour with $20k monthly revenue bonus threshold, then 25% of collections after

Percentage of monthly collections

• 26% up to $50,000 monthly revenues
• 27% for $50,000 – 65,000 in monthly revenues
• 28% for $65,001 in monthly revenues

Standard Hygienist Benefit Options (Based on Full Time status, pro-rated for less than Full Time.)

• Annual CE, license fees and other professional fee allowance of $250

Joining Bonus (Choose one. Based on Full Time status, pro-rated for less than Full Time. Not applicable for current team members)

• $5,000 for hygienist who commits to 2 years when signing agreement
  • $2,000 within first 30 days
  • $1,500 on 1-year anniversary
  • $1,500 on 2-year anniversary
  • Notice provided any time before 2nd anniversary will require 75% repayment of bonus paid

Annual Incentive for Full Time Team Members

• Opportunity to earn 10% of amount you exceed the previous year’s annualized individual revenues by $10,000.
  • Example: Exceed 2023’s revenue amount by $30,000 and earn a bonus of $2,000
  • Requirements: based upon full calendar year of service, no change in status in either comparison year

Additional Benefit Options (Based on Full Time status, pro-rated for less than Full Time. Ability to choose 2)
